Sullins Dee Freeman Governor Director Secretary June 11, 2009 CERTIFIED MAIL: RETURN RECEIPT REQUESTED Mr. Michael Coates Crossroads Development Corporation 12400 NC 50 N Willow Springs, NC 27592 Subject Property: Coates Asphalt and Grading Asphalt Plant Pole Cat Branch [030402, 27-46, C, NSW] REQUEST FOR MORE INFORMATION Dear Mr. Coates: DWQ Project # 09-0107, Ver. 2 Johnston County On June 5, 2009, the Division of Water Quality (DWQ) received your application dated January 26, 2009, to fill or otherwise impact 100 linear feet of perennial stream, and 5,086 square feet of Zone 1 Neuse River basin protected riparian buffers and 3,136 square feet of Zone 2 Neuse River basin protected riparian buffers to construct the proposed asphalt plant at the site. The DWQ has determined that your application was incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the additional information requested below, we will place this project on hold as incomplete until we receive this additional information. If we do not receive the requested information, your project will be formally returned as incomplete. Please provide the following information so that we may continue to review your project. Additional Information Requested: 1. Please locate all isolated or non-isolated wetlands, streams, and other waters of the State as overlays on the site plan. 2. Please locate all of the protected riparian buffers as overlays on the site plan, clearly showing Zone 1 and Zone 2. 3. Please show contour lines down slope of the level spreader to show that it has been correctly sited per the BMP Manual. 4. Please provide calculations to show the outlet velocity from outlet(s) detention pond scour hole to the wetland(s). The inventory should include all proposed building footprints, roads, driveways, sidewalks, gravel-surfaced areas, amenity areas, etc. A Stormwater Management Plan (SMP) must be provided for any drainage areas that exceed 24 percent imperviousness. For each BMP, provide a completed BMP Supplement Form with all the required items (see http://h2o.enr.state.nc.us/su/bmp forms.htm). Per NCAC 2B 15A: 02H.1000, a "drainage area" is defined as "the entire area contributing surface runoff to a single point." 6. Please complete Sections B-4 and F-3 of the PCN application. 7. Please provide stream determination documentation on the site (upstream and downstream photos and DWQ stream identification forms). USGS topographic map shows 2 streams and Johnston County soils map shows 3 streams. You show that the buffer stops on the east side of the property. 8. Will these be temporary riparian buffer impacts due to installation of the dissipater pad? If so, please enumerate them on the plans. 9. There appears to be a rip-rap pad on the downstream side of the pipes. It is unclear why this is necessary as the pipes are at 1.0 percent slope. The pipe length is reported as 44 linear feet and should probably be the impact unless downstream pad can be justified. Please submit this information within 30 calendar days of the date of this letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your project will be withdrawn and you will need to reapply with a new application and a new fee. This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please be aware that any impacts requested within your application are not authorized (at this time) by the DWQ.
