This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in Collection System Permit No. WQCS00247. A summary of the findings and comments noted during the inspection are provided in the enclosed copy of the inspection report. The Collection System Non-sampling inspection was conducted by Division of Water Resources staff from the Washington Regional Office. The following violation(s) were noted during the inspection: Inspection Area Description of Violation Inspections Pump Station inspection logbooks are incomplete. Continuous and routine logbook entries for pump station inspections are not being conducted. NOV-2018-PC-0321 Grifton Collection System August 15, 2018 Page 2 Inspection Area Description of Violation Operation and Maintenance There were no system cleaning records available for review at the time of the inspection. Operation and Maintenance The required annual cleaning of at least 10% of the gravity sewer system is not being conducted. Operation and Maintenance The facility has no portable, back-up power source for those pump stations not equipped with an on-site, permanent generator. Monitoring and Reporting Annual Report is not being prepared and distributed, as required. Pump Station #13 The emergency contact information signed must be relocated so that it will be easily visible to residents and passersby. Pump Station #14 The on-site permanent generator would not start. The system does not have a portable generator or portable pump available which can be used as a back-up power source. Pump Station #15 The high-water alarms did not work. There was no high-water alarm float present in the wet well at the time of the inspection. Pump Station #15 The telemetry system did not call when the test was conducted by turning off the pump station's power to simulate power outage. Pump Station #18 The high-water visual alarm light is located behind the emergency contact information sign and is not sufficiently visible. Either the high-water visual alarm light or the emergency contact information sign must be relocated. Inspections: Right-of-Way/ Right-of-Way/Remote Area is not being maintained. Located near Country Club Road and Fairway Drive near the old Grifton Country Club clubhouse, this Right-of-Way/Remote Area is completely overgrown limiting visual inspection and is not accessible in the event of an emergency. No inspection records were available for review at the time of the inspection. It appears that this Right-of-Way has not been maintained for several years. A Notice of Violation/Intent to Issue Civil Penalty is being issued for the noted violations of North Carolina General Statute (G.S.) 143-215.1 and the facility's Collection System Permit. Pursuant to G.S. 143-215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of any permit issued pursuant to G.S. 143-215.1. ®EQ North Carolina Department of Environmental Quality j Division of Water Resources 943 Washington Square Mall,Washington,NC 27889 252-946-6481 NOV-2018-PC-0321 ' Grifton Collection System August 15, 2018 Page 3 If you wish to provide additional information iregarding the noted violation, request.technical assistance, or discuss overall compliance please respond in writing within ten (10) business days after receipt of this Notice. A review of your response will be considered along with any additional information provided. You will then be notified of any civil penalties that may be assessed regarding the violations. If no response is received in this Office within the 10-day period, a civil penalty assessment may be prepared. Remedial actions should have already been taken to correct these problems and to prevent further occurrences in the future. The Division of Water Resources may pursue enforcement action for these and any additional violations of State law. If these violations are of a continuing nature, not related to operation and/or maintenance problems, and you anticipate remedial construction activities, then you may wish to consider applying for a Special Order by Consent.
