Sullins Dee Freeman Governor Director Secretary June 9, 2009 DWQ EXP # 09-0119v2 Guilford County Mr. Andrew LaRowe, Executive Director Facilities and Construction Management Guilford County Schools 617 West Market Street Greensboro, NC 27401 Subject Property: Jamestown Middle School APPROVAL OF STORMWATER MANAGEMENT PLAN (SW) Dear Mr. LaRowe: On May 22, 2009, the Division of Water Quality (DWQ) issued a 401 Water Quality Certification to you and Guilford County Schools to place fill within or otherwise impact 130 linear feet of an unnamed stream tributary to Deep River and 0.31 acres of wetlands, for the purpose of constructing a road crossing for the proposed "Jamestown Middle School" project in Guilford County. On May 28, 2009, the 401 Oversight/Express Review Permitting Unit of the Division of Water Quality (DWQ) received your application dated May 20, 2009, and a stormwater management plan (SMP) dated May 12, 2009, from Mr. Steven Miller of CLH Design, PA for the purpose of meeting Condition 5 of the 401 Certification for this project. On May 29, 2009, and June 5, 2009, the DWQ received revised SMPs dated May 29, 2009 and June 5, 2009 respectively, and on June 9, 2009, the DWQ received a revised Operation and Maintenance Agreement for Wet Pond 2. After reviewing the revised SMP, the DWQ approves the final version dated June 5, 2009 along with Sheet 3.4 in the plan dated May 29, 2009 to satisfy the stormwater condition required by the Water Quality Certification. You are required to meet the following conditions: The stormwater management system consisting of four wet detention ponds, all associated appurtenances, stormwater conveyances, and the grading and drainage patterns depicted on the plans, as approved by this office must be maintained in perpetuity. No changes to the structural stormwater practices shall be made without written authorization from the Division of Water Quality. The stormwater management structures shall have maintenance access and at least a 10- foot drainage easement on each side to allow for the ability to maintain the structures, and to perform corrective actions. The stormwater easements shall be recorded to provide protection of the structures from potential alterations by future property owners. 2.
