This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc) that can be administratively renewed with minor changes, but can include
facilities with more complex issues (Special Conditions, 30371istwater, toxicity testing,
instream monitoring, compliance concerns).

This facility has been permitted in the past as a discharge from a greensand filter system.
RRO inspected the facility in September 2012 and found the facility to be not exactly as
had been permitted previously. The installed system is an AdEdge filter treatment system.
It's essentially a greensand filter system, but the facility installed on site is designed to
operate as a 100 % recycle system, with backwash water returning to the head of the
WTP and backwash solids removed.
The permit has been drafted to reflect the system's design and operation, considering that
discharge may occur very infrequently, and only under extraordinary conditions. The
permittee describes these conditions as "problems that preclude use of backwash water."
If discharge does occur, the Permittee expressed confidence that the system will meet
greensand falter backwash quality, noting that solids -slurry is removed regularly from the
system. As a result, the monitoring frequencies for flow, TSS, pH, TRC and turbidity
have been set as "episodic," with a note stating the facility does not need to monitor more
than twice per month, should multiple discharges occur during any particular month; no
other changes recommended.
