Dear Mr. Whitt:
On November 17, 2006, the Division of Water Quality (DWQ) received your application dated
November 1, 2006, to impact 0.0.44 acres of 404/wetland, 985 linear feet of intermittent
important stream and 1,2301inear feet of intermittent unimportant stream, to construct the
proposed quarry expansion at the site. The DWQ has determined that your application was
incomplete and/or provided inaccurate information as discussed below. The DWQ will require
additional information in order to process your application to impact protected wetlands and/or
streams on the subject property. Therefore, unless we receive five copies of the additional
information requested below, we will have to move toward denial of your application as required
by 15A NCAC 2H .0506 and will place this project on hold as incomplete until we receive this
additional information. Please provide the following information so that we may continue to
review your project.
Additional Information Requested:
1. Please contact the DWQ to schedule intermittent/perennial stream determinations at the
site.
2. Please specify the percent of project imperviousness area and provide an inventory of all
impervious surfaces.
3. You indicated during our field meeting that bridging the perennial stream you are
proposing to culvert would not be feasible due to the weight of the haul trucks. Please
provide documentation supporting this assertion or alternative avoidance and
minimization for this crossing.

4. The DWQ recommends utilization of the new level spreader guidelines for your proposed
level spreaders.
5. In regards to your stormwater plan, please specifically compute drawdown time, stating
"a few days" is not specific enough.
6. Please submit 401 BMP worksheets.
7. Points B, C and D which are summarized in your table within section, "Water Quan.
Calcs. " of your Stormwater Impact Analysis need to be shown on plan sheets, not simply
shown on marked-up topographic maps.
Please re-submit your stormwater plan sheets at a scale of no smaller than 1"=50'.
Please respond within three weeks of the date of this letter by sending this information to me in
writing and Daryl Lamb of the DWQ Winston-Salem Regional Office. If we do not hear from
you within three weeks, we will assume that you no longer want to pursue this project and we
will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ. Please call Ms. Cyndi Karoly or Mr. Ian
