The Division has reviewed your application, received on February 23, 2018, for renewal of NPDES
permit NC0074306. To enable us to complete our review in accordance with N.C.G.S. 143-215.1
and 15A NCAC 021I .0105,we need additional or revised information regarding your discharge:
1. Please verify the outfall coordinates and name of the receiving stream.
2. Please verify the design discharge volume. This information is requested to verify the
maximum potential volume of discharge in the event that a discharge does occur.
3. Please verify the sludge management plan. Inconsistent information was provided with the
permit renewal application.
4. Please provide a schematic that shows the flow through the entire facility, including flow
volumes at all points as well as where chemicals are added for treatment of both potable
water and wastewater. This information was not provided with the renewal application.
