7. ANY CUT AND FILL QUANTITIES AS LISTED HEREON SHALL BE CONSIDERED
APPROXIMATE ONLY AND ARE SUBJECT TO CHANGE DEPENDING ON ACTUAL SITE
CONDITIONS AND CONSTRUCTION OPERATIONS. THE CONTRACTOR IS RESPONSIBLE
FOR VERIFYING QUANTITIES THROUGH A FIELD VISIT AND HIS/HER OWN QUANTITY
TAKE-OFFS.
8. CONTRACTOR IS TO REPLACE ANY DAMAGED STORM DRAINAGE WITH SAME SIZE RCP.
9. CONTRACTOR TO PROVIDE ADEQUATE SUPPORT FOR POWER POLES. COMPACT TRENCH
FIVE (5) FEET EACH SIDE OF POWER POLES TO 95%-98% STANDARD PROCTOR
10. RESTORE ALL DRIVES, FENCING, PAVEMENT, AND GRASSED AREAS TO THEIR ORIGINAL
CONDITION AND MAINTAIN UNTIL FINAL ACCEPTANCE OF THE PROJECT.
11. ALL ITEMS THAT ARE REMOVED AND REPLACED OR DISTURBED DURING
CONSTRUCTION ARE TO BE REPLACED WITH EQUAL OR BETTER QUALITY THAN THE
ORIGINAL.
12. WHEN EXISTING UTILITY LINES ARE EXPOSED DURING CONSTRUCTION THE
CONTRACTOR SHALL PROVIDE SUPPORT AS NECESSARY TO PREVENT DAMAGE TO THE
EXISTING UTILITY. ANY UTILITY DAMAGED SHALL BE IMMEDIATELY REPLACED AT NO
COST TO OWNER.
13. ROADS IN WORK AREA TO REMAIN CLEAR AT ALL TIMES. CONTRACTOR SHALL REPAIR
ANY PAVEMENT DAMAGED, AND ROADS IN WORK AREA TO BE CLEAN AT END OF EACH
DAY AT THEIR OWN EXPENSE.
14. NOT ALL TREES MAY HAVE BEEN SHOWN. MORE SIGNIFICANT TREE HAVE BEEN
SHOWN FOR CONTRACTOR CONVENIENCE.
15. CONTRACTOR SHALL MAINTAIN ACCESS TO AFFECTED PROPERTIES AT ALL TIMES
DURING CONSTRUCTION.
16. ANY EXISTING AC WATERLINE THAT IS DISTURBED SHALL BE HANDLED AND DISPOSED
OF IN ACCORDANCE WITH ALL LOCAL, STATE, AND FEDERAL LAWS.
17. WHERE GUY WIRES ARE TO BE DISTURBED BY THE MAIN INSTALLATION THE
CONTRACTOR SHALL CONSULT WITH THE AFFECTED UTILITY TO PROVIDE SUPPORT,
RELOCATIONS, OR REPAIRS AS NEEDED FOR WATER MAIN INSTALLATION.
CONTRACTOR SHALL BEAR ALL COSTS ASSOCIATED.
18. APPROVAL OF THIS PLAN SHALL NOT CONSTITUTE AUTHORIZATION TO BEGIN SYSTEM
CONSTRUCTION. ALL APPLICABLE PERMITS MUST BE OBTAINED PRIOR TO
CONSTRUCTION.
19. CONTRACTOR SHALL PROVIDE ALL MATERIALS AND MATERIAL CERTIFICATIONS FOR
THIS SITE UTILITY SYSTEM IN ACCORDANCE WITH THE APPROVED DRAWINGS AND
SPECIFICATIONS.
20. CONTRACTOR IS RESPONSIBLE FOR CONTROL OF WATER THROUGHOUT THE
CONSTRUCTION SITE, INCLUDING RUNOFF THROUGH DISTURBED AREAS.
21. THE TEMPORARY STREAM BYPASS PUMPS AND PIPING SHALL BE CONTAINED WITHIN
THE WORK AREA DEFINED BY SILT FENCE AND TREE PROTECTION FENCING UNLESS
THE CONTRACT DRAWINGS EXPLICITLY IDENTIFY AN ALIGNMENT FOR THE TEMPORARY
PUMPS AND PIPING OUTSIDE OF THESE AREAS.
22. IN-STREAM WORK AREAS MUST BE STABILIZED DAILY ONCE THE PUMP-AROUND IS
REMOVED OR DE-ACTIVATED AT THE END OF THE WORKDAY. DISTURBED AREAS
ABOVE THE WATERLINE SHALL RECEIVE STRAW/MULCH/COIR MAT ONCE STREAM FLOW
IS RE-ESTABLISHED AT THE END OF THE WORKDAY. THE CONTRACTOR SHALL ALLOW
SUFFICIENT TIME TO INSPECT THE NEW FLOW PATTERN AND MAKE ADJUSTMENTS AS
NECESSARY TO ENSURE NON-EROSIVE CONDITIONS BEFORE VACATING THE SITE.
23. CONTRACTOR IS RESPONSIBLE FOR ENSURING SMOOTH TRANSITIONS AT UPSTREAM
AND DOWNSTREAM ENDS OF WORK AREAS AND BETWEEN THE STREAMBED AND ITS
BANKS.
24. VOIDS SHOULD NOT BE LEFT IN ANY COMPLETED STREAM STRUCTURES. STRUCTURES
SHOULD BE FILLED IN WITH CHINKING STONE, STREAM BED MATERIAL AND/OR
ORGANIC MATERIAL (TOP SOIL).
25. ALL DISTURBED AREAS WITHIN THE LIMIT OF DISTURBANCE (EXCEPT CHANNEL) SHALL
BE RE-SEEDED AND/OR RE-PLANTED. ALL PLANTING AREAS ARE TO BE COVERED WITH
COIR MAT. INSTALL A WELL-MIXED COMBINATION OF THREE INCHES OF TOP SOIL AND
THREE INCHES OF COMPOST ONTO ALL GRADED AREAS REQUIRING COIR MAT. INSTALL
STRAW ON TOP OF THIS MIXTURE BEFORE INSTALLING AND STAKING COIR MAT.
26. SPECIFIED AND/OR PROPRIETARY PRODUCTS SHOWN HEREON MAY BE SUBJECT TO
SUBSTITUTION WITH OTHER PRODUCTS OF EQUAL OR SUPERIOR QUALITY AS
RECOMMENDED BY THE CONTRACTOR SUBJECT TO THE WRITTEN PREVIEW AND
APPROVAL OF THE DESIGN ENGINEER. WHEN THE CONTRACTOR USES PROPRIETARY
PRODUCTS, IT THE CONTRACTOR'S RESPONSIBILITY TO VERIFY WITH THE
SUPPLIER/MANUFACTURER THAT IT WILL FUNCTION PER THE DESIGN OR THE GIVEN

FUNCTION.
27. DRY WEATHER STREAM BASE FLOW IS A FUNCTION OF WEATHER PATTERNS, TIME OF
YEAR, GROUNDWATER, SNOW MELT, UPSTREAM MAN-INDUCED RELEASES, AND OTHER
FACTORS AND THEREFORE WAS NOT ESTIMATED FOR THIS PROJECT. IT IS THE
CONTRACTOR'S RESPONSIBILITY TO CARRY OUT SITE RECONNAISSANCE TO
DETERMINE PUMP-AROUND NEEDS SUCH AS THE NUMBER AND SIZE OF PUMPS HE/SHE
WILL NEED TO SUCCESSFULLY COMPLETE THE PROJECT.
28. CONTRACTOR IS RESPONSIBLE FOR CREATING AND MAINTAINING A SAFE AND
APPROPRIATE TRAFFIC SAFETY PLAN AND IS RESPONSIBLE FOR MAINTAINING A SAFE
WORK SITE INCLUDING ADHERENCE TO LOCAL STATE AND FEDERAL REQUIREMENTS

CONSTRUCTION SEQUENCE
1. CONDUCT A PRE-CONSTRUCTION MEETING WITH CONTRACTOR, OWNER, AND DESIGN
ENGINEER. CONTRACTOR TO ARRANGE MEETING; ALL PARTIES REQUIRE SEVEN (7) DAYS
NOTICE. PRIOR TO MEETING, CONTRACTOR IS TO HAVE LIMITS OF DISTURBANCE AND TREE
PROTECTION FENCING STAKED OUT FOR EXAMINATION. CONTRACTOR RESPONSIBLE FOR
HAVING ALL UTILITIES MARKED IN THE FIELD.
2. CLEAR FOR PERIMETER SEDIMENT CONTROL DEVICES, TREE PROTECTION FENCING, AND/OR
SILT FENCE. INSTALL DEVICES AND FENCING.
3. ALL WORK ASSOCIATED WITH CROSSVANE IS TO BE COMPLETED "IN THE DRY." INSTALL
STREAM PUMP AROUND AND SAND BAG DIVERSION AS DENOTED ON THE CONSTRUCTION
DOCUMENTS. INSTALL PUMP-AROUND SYSTEM ONLY FOR AS MUCH WORK AS CAN BE
COMPLETED IN ONE DAY. PUMP-AROUNDS ARE ONLY TO BE OPERATED DURING NORMAL
WEEKLY CONSTRUCTION BUSINESS HOURS AND SHALL NOT BE OPERATED OVERNIGHT OR
DURING STORM EVENTS.
4. REMOVE AND RE-DEPLOY PUMP AROUND SYSTEM AS NEEDED.
5. EXCAVATE AREA ADJACENT TO RIGHT ABUTMENT FOOTER TO EXPOSE FOOTER.
6. INSTALL CONCRETE BAGS AND FILL WITH GROUT ALONG RIGHT ABUTMENT AS SHOWN ON
CONSTRUCTION DOCUMENTS. ENGINEER TO APPROVE INSTALLATION.
7. CONSTRUCT CROSSVANE AS SHOWN ON CONSTRUCTION DOCUMENTS. ENGINEER TO APPROVE
INSTALLATION.
8. PLACE TOP SOIL, SEED, AND COIR MATTING IN ALL DISTURBED AREAS AND AS NOTED ON
DRAWINGS. ALL DISTURBED AREAS SHALL BE RETURNED TO ORIGINAL OR BETTER CONDITION.
9. EROSION AND SEDIMENT CONTROL DEVICES SHALL REMAIN IN PLACE FOR THE DURATION OF
PROJECT. UPON PROJECT COMPLETION AND APPROVAL FROM ENGINEER, REMOVE REMAINING
EROSION AND SEDIMENT CONTROL DEVICES.

PUMP-AROUND AND SAND BAG COFFER DAMS AS DENOTED ON THE
CONSTRUCTION DOCUMENTS.
3. INSTALL PUMP-AROUND SYSTEM ONLY FOR AS MUCH LINEAR FOOTAGE OF
STREAM WORK WHICH CAN BE COMPLETED AND TEMPORARILY OR PERMANENTLY
STABILIZED IN ONE DAY. PUMP-AROUNDS ARE ONLY TO BE OPERATED DURING
NORMAL WEEKLY CONSTRUCTION BUSINESS HOURS AND SHALL NOT BE
OPERATED OVERNIGHT OR DURING STORM EVENTS.
4. ALL WORK TO BE IMMEDIATELY STABILIZED WITH TOP SOIL, SEED AND COIR MAT
AS SHOWN ON THE CONSTRUCTION DOCUMENTS.
5. REMOVE AND RE-DEPLOY PUMP AROUND SYSTEM(S) AS NEEDED.
6. INSTALLATION OF PERMANENT VEGETATIVE PRACTICES MAY COMMENCE
IMMEDIATELY ASSUMING PROPER WEATHER CONDITIONS AND GROWING
SEASON.
7. CONTRACTOR TO SCHEDULE AND CONDUCT FINAL SITE WALK. AFTER APPROVAL
BY OWASA, THE AS-BUILT SURVEY MAY BE SCHEDULED.
8. WITH SEDIMENT CONTROL INSPECTOR’S PERMISSION, CONTRACTOR TO REMOVE
SEDIMENT CONTROL FEATURES, RESTORE ANY DISTURBED AREAS TO ORIGINAL
OR BETTER CONDITION AND DEMOBILIZE.
1. CONTRACTOR TO POST EROSION CONTROL PERMIT ON JOB SITE AS REQUIRED.
2. PERIMETER SEDIMENT AND EROSION CONTROL MEASURES SHALL BE IN PLACE
BEFORE ANY LAND CLEARING OR DISTURBANCE ACTIVITIES MAY BEGIN.
3. A RAIN GAUGE SHALL BE MAINTAINED IN GOOD WORKING ORDER ON THE SITE
UNLESS ANOTHER RAIN MONITORING DEVICE HAS BEEN APPROVED BY THE NC
DIVISION OF WATER QUALITY.
4. THE CONTRACTOR SHALL FURNISH AND INSTALL ALL NECESSARY EROSION
CONTROL MEASURES WHETHER OR NOT SHOWN ON THE PLANS TO PROTECT
ADJACENT CREEKS, RIVERS, ROADWAYS, ETC. FROM SILTATION AND EROSION.
5. CONTRACTOR SHALL NOTIFY THE STATE LAND QUALITY SECTION (919) 791-4200
AT LEAST 72 HOURS PRIOR TO START OF CONSTRUCTION TO SCHEDULE A
PRE-CONSTRUCTION MEETING.
6. CONTRACTOR STAGING/STOCKPILING AND PARKING AREAS SHALL BE CONFINED
TO AREAS WITHIN THE LIMITS OF CONSTRUCTION.
7. ALL EROSION MEASURES SHALL BE INSPECTED WEEKLY AND WITHIN 24 HOURS
AFTER EACH RAINFALL EVENT OF 12 INCH OR GREATER OF RAIN. MEASURES SHALL
BE REPAIRED OR REPLACED AS NECESSARY.
8. ALL EROSION CONTROL MEASURES SHALL BE CLEANED OF ACCUMULATED
SEDIMENT ONCE THE MEASURE HAS BECOME HALF FULL BY VOLUME.
9. SEDIMENT SHALL BE DISPOSED OF IN AN APPROVED MANNER SUCH THAT
FURTHER SEDIMENT TRANSPORT DOES NOT OCCUR. OFFSITE DISPOSAL AREA, IF
USED, SHALL HAVE ALL NECESSARY LOCAL AND STATE PERMITS.
10. ALL EXCESS SOIL RESULTING FROM INSTALLATION OF RAMPS SHALL BE REMOVED
FROM SITE BY THE CONTRACTOR. ANY SOILS TAKEN OFF SITE BECOMES THE
CONTRACTORS RESPONSIBILITY AND SHALL BE STOCKPILES AT LOCATIONS WITH
ALL REQUIRED STATE AND LOCAL PERMITS.
11. ALL EROSION CONTROL MEASURES SHALL REMAIN IN PLACE UNTIL ENTIRE SITE
IS COMPLETELY STABILIZED WITH VEGETATION AS PER SEEDING SPECIFICATIONS
AND SCHEDULE. ALL TEMPORARY EROSION CONTROL DEVICES ARE TO BE
REMOVED AFTER COMPLETE STABILIZATION HAS BEEN ACHIEVED.
12. THE CONTRACTOR SHALL RESTORE ALL DISTURBED AREA INCLUDING
DITCHES/SWALES TO ORIGINAL CONTOUR AND SLOPE AND TO ORIGINAL OR
BETTER CONDITION, EXCEPT WHERE NOTED ON THE CONTRACT DRAWINGS.
13. TOTAL ACREAGE DISTURBED = 0.12 ACRES.

ALL EROSION AND SEDIMENTATION CONTROL MEASURES SHALL BE INSPECTED
WEEKLY AND AFTER EACH RUNOFF PRODUCING RAINFALL. STRUCTURES THAT
SHALL BE INSPECTED INCLUDE:

ANY FABRIC WHICH COLLAPSES, TEARS, DECOMPOSES OR BECOMES INEFFECTIVE
SHALL BE REPLACED IMMEDIATELY. SEDIMENT DEPOSITS SHALL BE REMOVED
WHEN SEDIMENT BEHIND THE FENCE REACHES SIX INCHES.

SEEDED AREAS SHALL BE INSPECTED FOR FAILURE AND NECESSARY REPAIRS
AND RE-SEEDINGS SHALL BE MADE IMMEDIATELY, OR WITHIN THE SAME SEASON,
IF POSSIBLE.
4.
INSPECT CONSTRUCTION ROAD SURFACE REGULARLY, MAINTAIN IN A CONDITION
TO PREVENT SEDIMENT FROM LEAVING THE SITE, AND TOPDRESS WITH
ADDITIONAL STONE WHEN NEEDED. SWEEP EXISTING ROADWAY TO REMOVE ANY
ACCUMULATED SEDIMENT. SEDIMENT MAY BOT BE WASHED FROM ROADWAYS.

REMOVE SEDIMENT FROM AROUND INLET AFTER EACH STORM. CLEAN OR
REPLACE PROTECTIVE MEASURES AS NEEDED.
THE SEDIMENTATION POLLUTION CONTROL ACT WAS AMENDED IN 2006 TO REQUIRE
THAT PERSONS RESPONSIBLE FOR LAND-DISTURBING ACTIVITIES INSPECT A PROJECT
AFTER EACH PHASE OF THE PROJECT TO MAKE SURE THAT THE APPROVED EROSION
AND SEDIMENTATION CONTROL PLAN IS BEING FOLLOWED. RULES DETAILING THE
DOCUMENTATION OF THESE INSPECTIONS TOOK EFFECT OCTOBER 1, 2010. THE
SELF-INSPECTION PROGRAM IS SEPARATE FROM THE WEEKLY SELF-MONITORING
PROGRAM OF THE NPDES STORMWATER PERMIT FOR CONSTRUCTION ACTIVITIES. THE
FOCUS OF THE SELF-INSPECTION REPORT IS THE INSTALLATION AND MAINTENANCE
OF EROSION AND SEDIMENTATION CONTROL MEASURES ACCORDING TO THE
APPROVED PLAN. THE INSPECTIONS MUST BE CONDUCTED AFTER EACH PHASE OF THE
PROJECT, AND CONTINUED UNTIL PERMANENT GROUND COVER IS ESTABLISHED IN
ACCORDANCE WITH NCGS 113A-54.1 AND 15A NCAC 4B.0131. THE SELF-INSPECTION
REPORT FORM IS AVAILABLE AS AN EXCEL SPREADSHEET FROM:

1. CONTRACTOR SHALL PREPARE ALL AREAS TO RECEIVE TEMPORARY OR
PERMANENT SEEDING MEASURES PRIOR TO PLANTING.
2. TOPSOIL SHALL BE PLACED IN AREAS TO BE SEEDED AND ROUGHENED WITH
TRACKED EQUIPMENT OR OTHER SUITABLE MEASURES. SLOPES STEEPER THAN
3:1 MAY BE ROUGHENED BY GROOVING, FURROWING, TRACKING, OR STAIR-STEP
GRADING. SLOPES FLATTER THAN 3:1 SHOULD BE GROOVED BY DISKING,
HARROWING, RAKING, OPERATING PLANTING EQUIPMENT ON THE CONTOUR.
3. SOIL AMENDMENTS INCLUDING, BUT NOT LIMITED TO, LIME AND FERTILIZER
SHALL BE SPREAD AS NECESSARY, AND AT THE RATES SHOWN IN THE SEEDING
SCHEDULE. SEEDING SHALL BE AS PER TYPE AND RATES SHOWN IN THE
SEEDING SCHEDULE. SEED SHALL BE BROADCAST AS SOON AS POSSIBLE
FOLLOWING ROUGHENING, BEFORE SURFACE HAS BEEN SEALED BY RAINFALL.
1. MULCH MUST COVER A MINIMUM OF 80 PERCENT OF THE SOIL SURFACE AND
MUST BE SECURED BY TACKING, CRIMPING, OR NETTING.
2. WOOD CELLULOSE FIBER MULCH SHALL BE USED IN HYDROSEEDING GRASS SEED
IN COMBINATION WITH FERTILIZERS AND OTHER APPROVED ADDITIONS.
3. NETTING WITH PLASTIC MESH AND/OR PLASTIC TWINE SHALL BE USED IN
WETLAND AND RIPARIAN BUFFERS TO PROTECT SMALL ANIMALS.
1. HYDROSEEDING SHALL BE CARRIED OUT IN THREE STEPS. STEP ONE SHALL
CONSIST OF THE APPLICATION OF LIME. IN STEP TWO THE SEED MIXTURE SHALL
BE MIXED WITH THE FERTILIZER, WOOD CELLULOSE FIBER MULCH, AND ANY
REQUIRED INOCULANTS AND APPLIED TO THE SEED BED. STEP THREE SHALL
CONSIST OF APPLICATION OF TOP DRESSING DURING THE FIRST SPRING OR
FALL, WHICHEVER COMES FIRST, AFTER STEP TWO.
2. INGREDIENTS FOR THE MIXTURE AND STEPS SHOULD BE DUMPED INTO A TANK
OF WATER AND THOROUGHLY MIXED TO A HOMOGENEOUS SLURRY AND SPRAYED
OUT UNDER A MINIMUM OF 300-350 POUNDS PRESSURE, IN SUITABLE
PROPORTIONS TO ACCOMMODATE THE TYPE AND CAPACITY OF THE HYDRAULIC
MACHINE TO BE USED. APPLICATIONS SHALL BE EVENLY SPRAYED OVER THE
GROUND SURFACE. THE CONTRACTOR SHALL FREE THE TOPSOIL OF STONES,
ROOTS, RUBBISH AND OTHER DELETERIOUS MATERIALS AND DISPOSE OF SAME
OFF THE SITE. THE BARE SOIL, EXCEPT EXISTING STEEP EMBANKMENT AREA,
SHALL BE ROUGH RAKED TO REMOVE STONES, ROOTS, AND RUBBISH OVER
4-INCH IN SIZE, AND OTHER DELETERIOUS MATERIALS AND DISPOSE OF SAME
OFF THE SITE.
3. NO SEEDING SHOULD BE UNDERTAKEN IN WINDY OR UNFAVORABLE WEATHER,
WHEN THE GROUND IS TOO WET TO RAKE EASILY, WHEN IT IS IN A FROZEN
CONDITIONS, OR TOO DRY. ANY BARE SPOTS SHOWN IN TWO TO THREE WEEKS
SHALL BE RECULTIVATED, FERTILIZED AT HALF THE RATE, RAKED, SEEDED, AND
MULCHED AGAIN BY MECHANICAL OR HAND BROADCAST METHOD ACCEPTABLE TO
THE OWNER.
