Reference: February 4, 2009 Application for Section 404 Nationwide Permit 33 Dear Sir: The North Carolina Department of Transportation (NCDOT) proposes to replace Bridge No. 99 over Long Creek on SR 1968'. In addition to the 0.05 acre (49 linear feet) of temporary stream impacts requested in the original permit application dated February 4, 2009, there will be 90 linear feet of permanent stream impacts and <0.01 acre (45 linear feet) of temporary stream impacts to a previously undocumented intermittent unnamed tributary to Long Creek. Therefore, final impacts total 0.05 acre (94 linear feet) of temporary stream impacts and 90 linear feet of permanent stream impacts on this project. The replacement of this inadequate structure will result in safer and more efficient traffic operations. 3e. At the time, the only impacts on this project were from the temporary causeways in Long Creek. Polly Lespinasse (NCDWQ) informed NCDOT in late February about the existence of a previously undocumented intermittent stream that would likely be impacted by the project. NCDOT went out to the site on 3/4/09 to fill out stream fortes and GPS the stream. Updated drawings were prepared and this application is a revision to the previous NW 33 and a request for a NW 23 and written 401 certification to account for the impacts to this intermittent tributary to Lang Creek. 6.
