Over the past two weeks, heavy rains have impacted the project area. There has been flooding along the
creeks and severe erosion of slopes along the entire length of the project. The sediment losses have been
cataloged by site personnel and the approx amounts of sediment loss reported to DWR thru the past weeks.
This ICA is issued due to heavy sediment losses, contractor not managing the site runoff and not completing
work in areas thru-out the project, specifically around permitted sites. The completion of erosion control needs
listed as Urgent on the NPDES inspection reports requires a 24 hour response to address the issue. The
contractor has not applied sufficient personnel over the past several weeks to address the issues identified
before or duirng the storms.
Clean up of the EC devices was started and has proceeded slowly due to both the manpower issues with the
contractor and the weather conditions. While efforts were begun to make a coordinated plan for repairs and
clean up, overall contractor response has been slow to develop.
At this time, NCDOT forces are developing and implementing a work plan to address the project erosion and
sedimentation control needs including but not limited to the following:
-Repair of eroded slopes, failed slope drains, reinstalling silt fence. Also recovery of accumulated sediment

along the toe of slopes and cleaning out of temporary silt ditches
-Repair and adjustment of slope drains along the roadway and grading roadway area and ditches to drain to
the slope drains; installation of temp diversions to manage stormwater runoff directing it to EC devices
-Cleanout and repair of the skimmer basins, Type B basins, drop inlet protection, ditches and toe of slope
areas - repair and replacment of silt fence and silt checks, wattles, etc. as needed.
-Removal of accumulated sediment from wetland areas, stream channels and offsite property areas
It is recommended that the contractor work with the NCDOT personnel to approach the repair and clean up in
a systematic manner. All available contractor forces should be directed to work on the repair and cleanup in
jurisdictioinal areas first, then non-jurisdictional areas afterwards.
Specific to Site 10: Storm water coming down the roadway bypasses all slope drains and drop inlet boxes.
This resulted in overwhelming the only operational slope drain at the structures on the right side of the L-line.
Drop inlets were raised for paving. Weeps need to be knocked out or blocks removed. Sand bag diversions or
similar should be used to divert runoff into the drainage structures in the median and along the right side and
any remaining slope drains along the length of the roadway from str's 3&4 up to W Mountain St. Best would be
to finish the paving but in the interim, the runoff should be diverted all along the section to prevent blowing out
the slope drain at the bridges.
Specific to Site 24: Install and maintain a substantial ditch in front of and behind the noise wall to carry storm
water to the skimmer basins. Close off the wetland with properly installed silt fence and close off the spillway
area of the basin by connecting silt fence from the corner of the existing fence to the skimmer spillway berm.
At all times, general efforts to divert, direct, slow and contain sediment laden storm water must be taken.
Utilize temporary diversions, temporary silt ditches, intentional grading to shed runoff to EC devices. Utilize
staged final grading and permanent seeding to stabilize slopes and ditches.
Overall, the contractor should focus on completing work in an area to avoid the continuous maintenance and
impacts from erosion.
The follow up review for the project will be done by August 16.
