Subject: Request for Permit Modification to DWQ# 04-1684 (M.AIOI66R -Union County)
Dear Mr. Dorney:
In a request dated October 11, 2004, the North Carolina Department of Transportation
(NCDOT) requested permission to improve the safety of NC 218 in Union County by paving two
feet of road shoulder in each travel lane from the intersection of US 601 north to the intersection
of NC 205. Alan W. Klimek, P. E. authorized this request in a letter dated November 10, 2004.
Based upon further review, it has been determined that our original request should be modified to include the replacement of a failing 60" x 40' corrugated metal pipe with concrete n
headwalls located at Sta. 262+15. The existing pipe would be replaced with a 103" x 71' x 60' M
corrugated metal pipe (arched) for an additional 40 feet of impacts. .<: z

As previously mentioned. NC 218 is located within the Yadkin River Basin (Hydrologic
Unit # 03040105), affiecting the drainage of unnamed tributaries to Crooked Creek, Grassy Creek.
Brandon Branch, Gold Branch, Richardson Creek and Crisco Creek. Based upon this revised
request, there will be approximately 296 linear feet of permanent impacts to the existing stream
channels as a result of the proposed construction (40 additional feet of new impacts).
Table I includes information related to site number, stream name, existing pipe length (ft),
existing pipe width (in), proposed replacement or extended pipe length (ft) and width (in).
