Should have records for any of the questions in this report or the SSO reporting form that are applicable. 4) The Town has failed to meet its required 10% gravity system cleaning for the last 2 years by a considerable amount. 5) The Town still doesn't have a complete map of its system and is 3 years past its 10 -year grace period. A Notice of Violation & Intent to Assess is being issued for the last 3 items. Describe: Comment: Town's FOG ordinance viewed via website should be updated -- staff indicated the minimum cleaning schedule was 90 days but the ordinance does not indicate a minimum; 30 days unless otherwise approved is the most common. They also indicated that some small places use under the sink units, but the ordinance prohibits them; perhaps should say upon approval. Town should have a file on each location showing what they have, its location, and the required cleaning minimum. Our office expects an average of 10% or greater per year over a 3 year span which is more lenient than the flat annual10% required by the permit. Need to update Spill Plan with new phone numbers, agency name, etc., to require bacti sampling for spills over 1000 gallons to surface water, and eliminate flushing with potable water or chlorine unless on a paved surface with no storm drains. Pump station inspection records should be kept by station with more detail versus all stations by week -- it will be easier to review notes etc by station. Comment: The DumDs aDDear to be ODeratina well considerina their aDDearance as thev could use some cleaning & painting. The sleds the pumps sit on are in bad shape and may need to be replaced soon, but no water leaks were observed. The pumps are cycling way too often and staff indicated it can be as often as every 1-3 min. Should be cycling no more than Whir ideally, thus needs more storage, change pumps to VFDs, or divert flow away from station.
