Location description: The project area for the Western Trail Stream Restoration Improvements project is located between
Western Trail and Firewood Trail, approximately 0.35 mile west of High View Road, in Greensboro, Guilford County, North
Carolina.
Description of projects area and activity: This verification authorizes the permanent discharge of dredged or fill material into a
total of 150 linear feet of stream channel, including culvert removal and construction of a concrete toe wall for bank
stabilization (33 linear feet), and restoration of stream channel using natural channel design techniques (117 linear feet),

Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the enclosed
Conditions, your application signed and dated 7/3/2018, and the enclosed plans "Western Trail Stream Restoration
Improvements Project P-05831-03" Sheets 1-10. Any violation of the attached conditions or deviation from your submitted plans
may subject the permittee to a stop work order, a restoration order, a Class I administrative penalty, and/or appropriate legal
action.
This verification will remain valid until the expiration date identified below unless the nationwide authorization is modified, suspended
or revoked. If, prior to the expiration date identified below, the nationwide permit authorization is reissued and/or modified, this
verification will remain valid until the expiration date identified below, provided it complies with all requirements of the modified
nationwide permit. If the nationwide permit authorization expires or is suspended, revoked, or is modified, such that the activity would
no longer comply with the terms and conditions of the nationwide permit, activities which have commenced (i.e., are under construction)
or are under contract to commence in reliance upon the nationwide permit, will remain authorized provided the activity is completed
within twelve months of the date of the nationwide permit's expiration, modification or revocation, unless discretionary authority has
been exercised on a case-by-case basis to modify, suspend or revoke the authorization.
Activities subject to Section 404 (as indicated above) may also require an individual Section 401 Water Quality Certification. You
should contact the NC Division of Water Resources (telephone 919-807-6300) to determine Section 401 requirements.
For activities occurring within the twenty coastal counties subject to regulation under the Coastal Area Management Act (LAMA), prior
to beginning work you must contact the N.C. Division of Coastal Management.
This Department of the Army verification does not relieve the permittee of the responsibility to obtain any other required Federal, State
or local approvals/permits.

COMPLETE IMPLEMENTATION OF SEDIMENT CONTROL MEASURES, BYPASS PRACTICES, AND ASSOCIATED CHANNEL
AND BANK CONSTRUCTION IN HE FOLLOWING SEQUENCE:
1, BEGIN WORK AT THE UPSTREAM SECTION AND PROCEED DOWNSTREAM, IN SOME CASES, WORK MAY BEGIN
DOWNSTREAM IF APPROPRIATE. HE SEQUENCE OF CONSTRUCTION MUST BE FOLLOWED UNLESS WRITTEN
APPROVAL FOR DEVIATIONS FROM ENGINEER IS OBTAINED. ONLY BEGIN WORK IN AN AREA WHICH CAN BE
COMPLETED BY THE END OF THE DAY INCLUDING GRADING ADJACENT TO THE CHANNEL. AT THE END OF
EACH WORK DAY, THE WORK AREA MUST BE STABILIZED AND HE STREAM BYPASS PUMPS AND SUCTION
PIPES REMOVED FROM HE CHANNEL. SANDBAG DIKES MAY BE LEFT IN PLACE UNTIL NO LONGER REQUIRED.
WORK SHALL NOT BE CONDUCTED IN THE CHANNEL DURING RAIN EVENTS.
2, SITUATE SANDBAG DIKES AT THE UPSTREAM AND DOWNSTREAM ENDS OF THE WORK AREA, PUMP STREAM
FLOW (CLEAN WATER) AROUND THE WORK AREA. DISCHARGE (CLEAN WATER) ONTO A STABLE VELOCITY
DISSIPATER MADE OF RIPRAP OR SANDBAGS.
3. PUMP WATER FROM THE WORK AREA TO A SEDIMENT FILTERING MEASURE SUCH AS A DEWATERING BASIN,
SEDIMENT BAG. OR OTHER APPROVED SOURCE, LOCATE HE MEASURE SUCH THAT THE WATER DRAINS RACE
INTO THE CHANNEL BELOW THE SEDIMENT SANDBAG DIKE.
4, AVOID TRAVERSING A CHANNEL REACH WITH EQUIPMENT WHERE NO WORK IS PROPOSED. IF EQUIPMENT HAS
TO TRAVERSE SUCH A REACH FOR ACCESS TO ANOTHER AREA, USE TIMBER MATS OR SIMILAR MEASURES
TO MINIMIZE DISTURBANCE TO THE CHANNEL. USE TEMPORARY STREAM CROSSINGS ONLY MEN NECESSARY
ONLY WHERE NOTED ON HE PLANS OR SPECIFIED,
5. INSTALL ALL STREAM RESTORATION MEASURES AS INDICATED BY THE PLANS. GRADE ALL BANKS IN
ACCORDANCE WITH THE GRADING PLANS AND TYPICAL CROSS-SECTIONS. STABILIZE ALL DISTURBED STREAM
BANKS AT THE END OF EACH WORK DAY WITH SEED AND MULCH OR SEED AND MATTING AS SHOWN ON
THE PLANS.
6. REMOVE CLEAN WATER DIKE AFTER ALL INSTREAM WORK IS COMPLETED AND STABILIZED, ESTABLISH A NEW
CLEAN WATER DIKE UPSTREAM FROM THE OLD SEDIMENT DIKE AFTER THE FIRST SEDIMENT FLUSH, REMOVE
11. IF CHANNEL BED EROSION IS OCCURRING, INCREASE THE MATERIAL OR PLACEMENT SIZE OF THE OUTFALL
PROTECTION.
12. REMOVE AND REPLACE DAMAGED OR LEAKING SANDBAGS.
14, CHESTERFIELD WILL WITHHOLD PAYMENT EQUIVALENT TO ALL FINES RESULTING FROM NONCOMPLIANCE WILF
APPLICABLE REGULATIONS.
15, STAY NITHIN THE UNITS OF HE DISTURBANCE AS SHOWN ON THE PLANS AND MINIMIZE DISTURBANCE
