There should be one upgradient MW and two downgradient MW’s. The map dated February 13, 2018
shows MW#1, #2 & #3. The map attached to the current permit shows MW#1, #2 & #4 with “proposed”
at each MW site. The map revision date is needed. The latitude & longitude of the established horizontal
control monument is missing. The well construction forms (GW-1) have not been submitted to the regional
office.

My responses to your questions are in red as follows:
1. Site map - #5 from the inspection letter – The site maps submitted (Sheet C-
301 & C-300) do not have a legend. All of the information from Permit Condition
I.4. should be shown on the map. See attached revised drawings.
Sheet C-301 – The Infiltration Basin is shown as proposed. Revised drawing
attached have dropped the word proposed.
Sheet C-300 – The Monitoring Wells (MW’s) are not labeled. MW#4 is not shown.
The information table at the bottom of the page shows Basin #C1, the basin
should be “C”. MW#4 was determined to be the future monitoring well. Since it
was not built we did not show it on the as-built map.
2. Groundwater Monitoring Well Construction Record (GW-1) - #6 from the
inspection letter. These records have not been submitted. The Owner sent
information form Greene Brothers last year. The Owner is verifying that GW-1
information was in the data sent previously and will resend all information
under separate cover.
3. Minor modification – From the inspection letter – this has not been submitted
to the Central Office in Raleigh. Item #2 (the change to a vegetated
infiltration basin) should be included in the modification. A new Engineer’s
Certification should be submitted. Being submitted by separate cover to Raleigh
Office by hard copy for review and revision.
4. The attached e-mail with the permit description indicates a 57,533 gpd
system. Please clarify the differences in flow references:
57,533 gpd; 17,533 gpd in current permit; 80,000 gpd on C-301 & C-300.
We did not mean this to be confusing. Upon completion of this high rate
infiltration disposal system, the wastewater plant for Trillium will have dual

permits. The surface discharge permit under which the upgraded Trillium
wastewater treatment plant is now operating is for 40,000 GPD. On receiving a
certificate operate for the high rate infiltration basin will be permitted for
17,533 GPD. The wastewater plant was designed and constructed for two phases
with the one being built as 80,000 GPD. This is the reason for the various
labeling.
5. The site map should meet the requirements of Permit (attached) Condition
Schedule I.4. An example (Mystic Meadows) site map is attached. Please note
per the permit, this was supposed to be submitted within 60 days of completion
of the monitoring wells. See attached revised drawing.

The Asheville Regional Office has received some of the information requested in
the attached inspection letter dated November 6, 2017. The following items need
clarification and in some instances still need to be submitted:

Dear Mr. Culbreth:
The North Carolina Division of Water Resources conducted an inspection of the Trillium Links &
Village WWTP on 9/30/2017. This inspection was conducted to satisfy permit Condition
Schedule I.2. (in-place inspection prior to operation) in Non-discharge Permit
No. WQ0037555. The findings and comments noted during this inspection are provided in the
enclosed copy of the inspection report entitled "Compliance Inspection Report".
Please respond in writing to this office within 30 days of your receipt of this letter regarding
your plans or measures to be taken to address the following issues:
These issues should be resolved prior to operation of the infiltration system.
1. The monitoring wells (MW) should have locks and ID tags.
2. Public access to the infiltration site is prohibited – please install whatever method
(signs/fencing) is determined to be effective in preventing access.
3. Install matting on the cut slope at MW #3.
4. Repair the audible/visual high water alarms at the WWTP.
5. Submit a site map (Permit Condition I.4.) Please include the closest water supply wells
on the site map.
6. Submit Groundwater Monitoring Well Construction Record (GW-1Forms).
A minor modification will need to be submitted by the permittee to address the following:

3. Submit the as-built field layout & certification.

The inspection was conducted by Beverly Price of the Asheville Regional Office. Assisting with the inspection were Tessa
Monday, Ashley Kabat and Sonja Gregory of the Non-Discharge Permitting Unit in Raleigh; Mike Beck, ORC with RPB
Systems; Robert Burgin, P.E. and Freddie Boan, Manager, Trillium Links & Village, LLC.
The inspection was conducted to satisfy Permit Schedule I.2. (in-place inspection) prior to start-up.
The following items should be addressed:
1.The monitoring wells (MW) should have locks and ID tags.
2.Public access to the infiltration site is prohibited – please install whatever method (signs/fencing) is determined to be
effective in preventing access.
3.Install matting on the cut slope at MW #3.
4.Repair the audible/visual high water alarms at the WWTP.
