The ORC, Mr. Dan Fortin was very helpful and demonstrated repairs to date and discussed plans for other repairs and modifications. The items mentioned in the previous inspection such as the electrical service and wiring, tertiary filter media replacement and aeration blower repairs have all been completed. Additional work is planned for the alarm and notification system. The mixed liquor concentrations between both aeration basins appeared much better with even concentrations in each basin. The effluent was clean with no visible solids, the table chlorinator used for disinfection was in service, the infiltration basins were free of ponding, algae or vegetation and the flow meter had recently been calibrated. (if no, note below.) 0 ❑ ❑ ❑ Comment: All treatment process were operable and progress has been made with blower repair, tertiary filter media replacement, electrical supply and aeration basin biomass. Treatment Flow Measurement -Influent Yes No NA NE Is flowmeter calibrated annually? ❑ ❑ 0 ❑ Is flowmeter operating properly? ❑ ❑ 0 ❑ Does flowmeter monitor continuously? ❑ ❑ 0 ❑ Does flowmeter record flow? ❑ ❑ 0 ❑ Does flowmeter appear to monitor accurately? ❑ 1:10 ❑ Comment: No comment Treatment Flow Measurement -Water Use Records Yes No NA NE Is water use metered? ❑ ❑ 0 ❑ Are the daily average values properly calculated? ❑ ❑ 0 ❑ Comment: No comment Treatment Flow Measurement -Effluent Yes No NA NE Is flowmeter calibrated annually? 0 ❑ ❑ ❑ Is flowmeter operating properly? ❑ ❑ ❑ Does flowmeter monitor continuously? ❑ ❑ ❑ Does flowmeter record flow? ❑ ❑ ❑ Does flowmeter appear to monitor accurately? ❑ ❑ ❑ Comment: Effluent flowmeter (Sigma 980) was last calibrated on April 14, 2018 by Delta Systems Environmental. Effluent flow rates during June 2018 have ranged from 6,300 gallons per day to as high as 19,700 gallons per day during the weekend. The generator is scheduled for immediate repair by Powerhouse generator service. The generator was tested by interrupting the incoming power supply, the generators' motor started as required but would not transfer power due to this issue. Treatment Activated Sludge Yes No NA NE Is the aeration mechanism operable? ❑ ❑ ❑ Is the aeration basin thoroughly mixed? ❑ ❑ ❑ Is the aeration equipment easily accessed? ❑ ❑ ❑ Is Dissolved Oxygen adequate? ❑ ❑ ❑ Are Settleometer results acceptable? ❑ ❑ ❑ Is activated sludge an acceptable color? 0 ❑ ❑ ❑ Comment: Both aeration basins appeared to have adequate mixed liquor concentrations with little foam. Aeration is supplied by two blowers that are controlled with on/off timers. The RAS and WAS flows are supplied from air lift pumps driven by the same blowers. 30 minute settleometer and effluent ammonia test strips are used to dictate process changes and wasting rates. Treatment Clarifiers Yes No NA NE Are the weirs level? ❑ ❑ ❑ Are the weirs free of solids and algae? ❑ ❑ ❑ Is the scum removal system operational? ❑ ❑ ❑ Is the scum removal system accessible? ❑ ❑ ❑ Is the sludge blanket at an acceptable level? ❑ ❑ ❑ Is the effluent from the clarifier free of excessive solids? 0 ❑ ❑ ❑ Comment: Clarifier compartments had effluent that was clear with little visible solids. Treatment Return pumps Yes No NA NE Are they in place? 0 ❑ ❑ ❑ Are they operational? 0 ❑ ❑ ❑ Comment: RAS and WAS flows are from air driven lift pumps Treatment Filters Yes No NA NE Is the filter media present? 0 ❑ ❑ ❑ Is the filter media the correct size and type? ❑ ❑ ❑ Is the air scour operational? ❑ ❑ ❑ Page: 4 DocuSign Envelope ID: 2CCE8953-216E-4892-896C-AEBD91 FA9227 Permit: WQ0003067 Owner - Facility: C & P Enterprises Inc Inspection Date: 06/06/2018 Inspection Type : Compliance Evaluation Reason for Visit: Routine Is the scouring acceptable? ❑ ❑ ❑ Is the clear well free of excessive solids? ❑ ❑ ❑ Is the mud well free of excessive solids and filter media? ❑ ❑ ❑ Does backwashing frequency appear adequate? 0 ❑ ❑ ❑ Comment: Media has recently been replaced in both units, in -pipe "leveler" have been added to evenly split flow rates between both of the filters. A separate blower supplies the air scour during backwashing. Treatment Disinfection Yes No NA NE Is the system working? High Rate ❑ ❑ ❑ Do the fecal coliform results indicate proper disinfection? ❑ ❑ ❑ ❑ ❑ ❑ Is there adequate detention time (-30 minutes)? ❑ ❑ ❑ ❑ ❑ Is the system properly maintained? ❑ ❑ ❑ ❑ ❑ If gas, does the cylinder storage appear safe? ❑ ❑ ❑ Is the fan in the chlorine feed room and storage area operable? ❑ ❑ ❑ Is the chlorinator accessible? ❑ ❑ ❑ ❑ ❑ ❑ If tablets, are tablets present? ❑ ❑ ❑ ❑ ❑ Are the tablets the proper size and type? ❑ ❑ ❑ ❑ ❑ ❑ Is contact chamber free of sludge, solids, and growth? ❑ ❑ ❑ If UV, are extra UV bulbs available? ❑ ❑ ❑ If UV, is the UV intensity adequate? ❑ ❑ ❑ # Is it a dual feed system? ❑ ❑ ❑ Does the Stationary Source have more than 2500 lbs of Chlorine (CAS No. 7782-50-5)? ❑ ❑ ❑ If yes, then is there a Risk Management Plan on site? ❑ ❑ ❑ If yes, then what is the EPA twelve digit ID Number? (1 000 ) If yes, then when was the RMP last updated? Comment: A 4 -tube tablet clorinator is used for effluent disinfection, slow dissolving calcium hypochlorite tablets suitable for wastewater effluent were being used.
