A preconstruction conference was held at the Ahoskie District Office on Friday, August 4, 2017 at
10:00 a.m.
Attendees and Proiect Personnel:
Representing the Prime Contractor, W.C. English, Inc. were Paul M. Johnson, Baxter Gordon, Billy
Williams, Kent Williams, Goutam Panda, Douglas Dalton, Bennie Davis and Alan Scott.
Representing NCDOT were Michael Hill, PE, Brooks Braswell, PE, James Stephenson,
Scott Emary, PE, Shawn Mebane, PE, Andy Blankenship, PE, Randy Hall, PE, John Abel, Maria Bonds,
Michelle Pittman and Clay Willis.
Representing the environmental agencies were Garcy Ward with NCDWR and Kyle Barnes with US Army
Corp. of Engineers.
Representing the utiliry companies were Bon Best with Lamberts Cable Splicing, Terry Clements with Century
Link, Robert Wood and James Pulley with Dominion.

Michael Hill opened the meeting and yielded the floor to Clay Willis and the environmental agencies to review
the permit conditions.
Kyle Barnes reviewed the Army Corps of Engineer's Permit. The permit needs to be kept onsite. Paul
Johnson said that there will be a box onsite to keep the permit.
Garcy Ward reviewed the Water Quality permit. Garcy was okay with the silt fence being outside of
the toe of slope in wetland areas. Clay wants the silt fence offset to be 5' from the toe of slope. In
these areas, the toe of slope needs to be well marked. Andy wants the contractor to quickly seed the
disturbed area behind the silt fence with native seed mix.
Andy Blankenship reviewed the erosion control portion of the contract. Mr. Blankenship emphasized
and encouraged the use of staged seeded. Crimping straw within 5 feet of the edge of pavement was
discussed.
Mr. Blankenship also discussed the approval process of borrow and waste site reclamation plans. He
emphasized that all material coming to or leaving from the project needs to originate or be delivered to
an approved site.
Michelle Pittman reviewed the Right of Way information. She noted that Mr. Morris on Parcel 1 could
be testy. She is in the process of getting the Lang house on parcel3 removed.
Issues•
Mr. Johnson stated that they plan to begin work on 8-7-17 with the initial surveying work.
Dominion Power will complete their work at the Y2 intersection in around 2 weeks. I am supposed to
notify Dominion if they need to supply power to the traffic signal at Y2.
Dominion will complete their work at the Yl intersection in around 3 weeks after the Y2 intersection is
complete. Time Warner Cable will relocate their lines in around 60 to 90 Days after pominion Power
is complete. CenturyLink said that they normally put up their line after Time Warner Cable, but they
may be able to put up their line before Time Warner Cable in this situation. CenturyLink was going to
check if they had any lines at Yl. Piedmont Natural Gas was supposed to be done with their relocation
today. AT&T will be done with their relocation work in about 2 weeks and plan to put the line in
service around 2 months after the relocation.
Mr. Hill requested that English report all DBE payments in the NCDOT tracking system.
Method III clearing allows 10' of clearing beyond the construction limits but the clearing should not
exceed the right of way or what is permitted.
The MSE wall design factor was still in question. Mr. Hill stated that he was waiting on an answer.
The existing traffic light will need to be maintained. Mr. Hill is working with NC Power to have power
supplied at the site for the light.
Mr. Hill stressed the importance of safety on the project, especially while working over traffic.

Certified payrolls will be required on this project.
Structure submittals should be sent to James Bolden and copy Emmanuel Omile, and Madonna Rorie.
Geotechnical submittals should be sent to Chris Kreider. Copy the Resident on all submittals.
Maria Bonds pointed out that mix designs should be sent to the Resident.
NCDOT will request the County Maintenance Engineer to remove the stockpile of material located at
Y2 if he wants it. If English construction wants this material to remain in place for their use, they are to
notify the Department by Monday morning.
The Department may request a bond for any secondary roads used to haul borrow.
The Department is going to request a supplemental agreement to replace the GRAU-350 guardrail end
units with GREU end units. Randy Hall noted that there are new guardrail specifications as well that
may need to be handled through a supplemental agreement.
Mr. Abel stated that the next phase of the R-5311 project is currently under design. Design and survey
work may occur within the project limits of C203941 related to the next phase of construction.
This completes the minutes of the Preconstruction Conference for Contract C203941 held on Friday,
August 4, 2017 at the Ahoskie District Office. Any discrepancies with the content of these minutes
should be submitted in writing to this office. Failure to do so will be an indication of agreement.
