The Divisinn of Wa[er Resources has reviewed your submittal for a 401 Water Quality Certifica[ion for [he
aforementioned project. Review of your application mvealed it lacking necessary information required for making
an informed permit decision. The perniit application was deficient in the following areas:
• Additional information provided after the on-sire review is unclear for new impacts in the pemiitted areas.
Please provide a total and complete list of impacts submitted with the original application and additional
stabilization impacts requested durin, the field review in one table for clariry.
• Fourth Creek is a 303D listed stream for mrbidity. Please describe extra measures to be taken to ensure
consfruc[ion will not negatively affect surface waters
Therefore, pursuant to I SA NCAC 2H .0507(a)(5), we will have to place the permit application on hold until we are
supplied the necessary information. You have 21 days ro respond in writing with the requested information or
notification to this office that the information is foithcomine. If, at the end of the 21 days, this office has not
received [his information in writing, we will assume you are wi[hdrawing your application and it will be retumed.
Furthermore, until [he information is received by [he NC Division of Water Resources, we reques[ (by copy ofthis
lettery that the US Army Corps of Engineers place the permit application on hold.
