The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater
Management Permit Application for the subject project on June 20, 2018. A review of that
information has determined that the application is not complete. The following information is
needed to continue the stormwater review:
1. Please submit a copy of the most recent deed to verify ownership of the site.
2. Please complete and submit a Supplement EZ form for the wet pond. This form can be
found at: https://deq.nc.gov/sw-bmp-manual.
3. Please complete and submit a high density residential subdivision deed restriction form.
This form can be found at: https:Hdeq.nc.gov/about/divisions/energy-mineral-land-

4. General MDC 9 requires all SCMs and associated maintenance accesses on privately
owned land except for those located on single family residential lots to be located in
permanent recorded easements. Please revise the plan to show the easement.
5. The two wet pond details on Sheet C404 show different crest elevations for the forebay
berm (624.5 vs. 623.5). Please revise the details to correct this discrepancy.
6. The average depth of the permanent pool, when calculated using the methods found on
Page 8 of the Stormwater Design Manual Wet Pond Chapter (C-3) is below the minimum
average depth of 3 feet.
7. The incremental storage in the wet pond main pool between El. 622.5 and El. 623.5 was
calculated incorrectly.
8. The SADA ratio used in the calculations was incorrect.
9. On the landscaping plan, please specify that the dam structure, including front and back
embankment slopes shall be vegetated with non -clumping turf grass and that trees and
woody shrubs shall not be allowed (Wet Pond MDC 11).
10. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other
documents. Pdfs must be uploaded using the form at:
https:Hedocs.deq.nc.gov/Fonns/SW ProjectSubmittal
Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to August 5, 2018, or the
application will be returned as incomplete. The return of a project will necessitate resubmittal
of all required items, including the application fee. Please reference the State assigned project
number SW3180605 on all correspondence.
If you need additional time to submit the information, please submit your request for a time
extension to the Division at the contact below. The request must indicate the date by which you
expect to submit the required information. The Division is allowed 90 days from the receipt of
a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an
approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject
to enforcement action pursuant to NCGS 143-215.6A.
