This is not an uncommon occurrence for a couple of reasons. One reason is that many contractors don't work a full schedule, or deploy a full staff on Fridays (some occasionally don't work at all) to allow their out of town employees to go home for the weekend, so there fewer people on the site to inspect each location and catch these things. If the rain event ends either late in the day, or at night on a Thursday it's a good possibility that a blowout won't be found until Monday, especially on a larger project like this one with many JD sites to check. Some sites are also not easily accessible right after a heavy rain event, which can also delay their inspection. Another issue specific to this project is that it is winding down. I would guess that the project is about 85% to 90% completed, with much of the inspectors attention focused on paving prep, or actual paving. The DOT and contractor construction trailers have been removed, and much of the inspection staff has been reassigned to other projects. The smaller inspection staff will take a little longer to go through the project to check all of the potential failures locations. The particular site in question was a stable site that had been completed a while ago; it was properly matted, and had already established a decent grass cover, so this wouldn't necessarily be the first site they would have inspected. The main thing is that they did find this one early Monday morning, and reported as soon as they found it. I forwarded the information to you when I came in from the field later that day and went through my email. We always try to report problems ASAP but, thing do get delayed or missed on occasion despite our best efforts. The Multi-Sensor Precipitation Estimate For July 12, 2018 is attached. The rainfall occurred on Thursday, July 12, 2018; however, it appears as though the sediment loss was not initially reported until Monday morning, July 16, 2018. Do you require the contractors to report the sediment loss to you within 24-hours? The reported rainfall was approx. 3" in two hours. The measures were in place but were overwhelmed. The soil in the area area has a very high sand content, and has always been unstable. The contractor has already started clean up. Please let me know if you need additional information. We had some silt loss at L Lt 427+00 where Basin 9.1F was removed and the rock lined ditch was installed. Silt fence was installed on the slope side to prevent silt washing into the rock lined ditch. Silt washed under the stone from the pipe outlet. We have approximately 3-4 cubic yards of material offsite in this area. I have attached photos of the silt loss. Forces are going to need to remove an area of controlled access fence and track down the slope in this area to retrieve the material. If you need any further information you can contact me.
