Signature: Date: 9---- 3 6 ---- / F FORM: FTA 04-16 Page 5 of 5 Skylars Cove Subdivision Sewer Extension Sanitary Sewer Narrative Description: Travel 1.69 miles north on Village Road from the overpass at US 74/76 (Andrew Jackson Hwy) and NC 133 (River Road). Turn left onto Gardenview Court and travel 0.19 miles to the site. The site is located on the west side of Gardenview Court. The purpose of this project is to extend the existing gravity sanitary sewer main in Creekview Estates from the cul-de-sac of Gardenview Court onto the adjacent property to the west of Creekview Estates, in order to be able to serve the adjacent property with gravity sanitary sewer. There is 113.93 LF of 8" public gravity sanitary sewer main being proposed for this project. The length is based on the distance from inside wall of manhole to inside wall of manhole. No flow is being permitted. A preliminary layout, which is subject to change, for the adjacent property shows 36 single-family residential lots. A separate sanitary sewer permit for flow and gravity main extension shall be obtained for the adjacent property once it is designed. 95.24 LF of the 101.51 LF of the proposed sanitary sewer main from SSMH-1 to SSMH- 2 is within 50 feet of wetlands. The sanitary sewer main from SSMH-1 to SSMH-2 will meet water main standards (15A NCAC 18C) for materials, testing methods and acceptability standards. SSMH-2 is within 50 feet of wetlands. SSMH-2 shall comply with the Division's draft separation requirements for manholes within 50 feet of wetlands. The proposed sanitary sewer main from SSMH-1 to SSMH-2 will be a "high priority sewer" line. Approximately 44.5 5 LF of the proposed sanitary sewer main will be an aerial line. The main becomes an aerial approximately 5.06 LF downstream of SSMH-2 (see provided plan profile). The proposed gravity sanitary sewer main will discharge into an existing sanitary sewer manhole at the cul-de-sac of Gardenview Court in Creekview Estates.
