The cooperation and assistance of John Pitts, Public Works Director, and Kenneth Kopp, collection system operator, during the inspection were greatly appreciated. Our records indicate the wastewater collection system consists of approximately 15.5 miles of gravity sewer, approximately 10.1 miles of force main, 9 duplex pump stations, and 2 high priority sewer lines. The subject permit was effective beginning April 1, 2013, and expired March 31, 2018. Section VI, item 6 of the subject permit requires the permittee to request renewal of the permit at least six months prior to the permit's expiration. Within thirty days of receiving this letter,please send two copies of the complete renewal application and any attachments to Steve Lewis,NCIJWR PER CS, 1617 Mail Service Center,Raleigh,NC 27699-1617. There is no fee to renew the permit. The items below show what conditions were noted during the inspection: 1. Reported Sanitary Sewer Overflows. During the two-year period ending May 31, 2018, there were no reported SSOs from the town's collection system. 2. Fats, Oils and Grease Program. The Town of Kenly, hereinafter referred to as the Town, has developed an educational fats, oils and grease program that includes educational material targeted at both residential and non-residential users. Due to recent staff changes, John Pitts could not locate documentation showing the educational flyers were distributed to customers in 2017. Section I, item 4 of the subject permit requires the permittee to distribute educational material to users at least bi- annually. Please submit written notice to this office when the educational material is distributed to customers in 2018, and include the method used to distribute the material. 3. Capital Improvement Plan. Section 1, item 5 of the subject permit requires the permittee to "adopt and implement a Capital Improvement Plan to designate funding for reinvestment into the wastewater collection system infrastructure." Town of Kenly, WQCS00173 June 22, 2018 Capital Improvement Plan. Within thirty days of receiving this letter,please provide this office with a copy of the Town's current Capital Improvement Plan. 4. Contingency Plan for Pump Failure. The Town maintains a contingency plan for pump failure at each lift station. In the event of a pump failure at a lift station, John Pitts stated the operators would remove the pump for repair and ensure the remaining pump is sufficient to run the station. 5. Pump Stations Inspected. The following duplex pump stations were inspected: Public Works Lift Station-(refer-red to-as-Railroad-Street-Lift Station-in the-permit);—Little River-Lift-Station-(referred-to-as--- 501 Johnston Parkway Lift Station in the permit); St. Mary's Lift Station(referred to as 1,200 St. Mary's Church Rd. in the permit); Glendale-Kenly School Lift Station (referred to as 400 Bay Valley Rd. in the permit); Goldsboro Street Lift Station; Yukon Street Lift Station; and Seventh Street Lift Station. The inspectors' observations noted in the next 5 items below refer to the seven pump stations inspected. Overall,the pump stations were well maintained. The wet wells were free of excessive debris and grease, and the inspectors did not observe any signs of recent overflow. The pump stations are not equipped with permanent generators. Standby power is available with a portable generator. Except for the Public Works Lift Station and the Glendale-Kenly School Lift Station, the pump stations visited were equipped with a quick hookup for a portable generator. The collection system operators inspect each pump station daily. 6. Pump Station Signage. Each pump station has signage clearly and conspicuously posted that identifies the pump station,provides an emergency 24-hour contact telephone number(s), and provides instructions for notification if the visual alarm illuminates and/or the audible alarm sounds, or if an emergency is apparent. 7. Pump Station Restricted Access. The pump station sites, equipment and components were observed to have restricted access. 8. Pump Stations Automatic Polling Feature. The pump stations are wired for simple telemetry, but are not connected and setup for the remote access. Use of the telemetry system would allow the collection system operators routine contact with the pump station from a central location to check the operational status of the system. 9. Pump Station High Water Alarms. The pump stations are equipped with audible and visual high water alarms. During the inspection,these alarms functioned properly. At the Public Works Lift Station, the visual alarm is not visible from the street. This office recommends relocating the visual alarm so that it is visible from the street. 10. Pump Station Inspection Program. The Town has developed and implements a daily routine inspection of each pump station that includes the following maintenance activities: cleaning and removing debris from the pump station area and wet well; inspecting all valves; inspecting pumps and other mechanical equipment; and verifying the proper operation of the alarms and auxiliary equipment. The inspector reviewed the inspection logs of the pump stations visited and observed adequate entries of the date and time of the operator's inspection, observations made, and any maintenance, repairs, or corrective actions taken. 11. Designated Operators. The Town's wastewater collection system is classified as a Grade 2 collection system(CS-2). Currently, the Town does not have a designated certified Operator in Responsible Charge (ORC). The back-up ORC on record is Kenneth Kopp. The certification type and status for Mr. Kopp is reflected as CS-2 and active, respectively, in the Division's data system. Due to recent staff changes, John Pitts stated the Town is in the process of getting some of their current operators certified for the collection system. North Carolina Administrative Code Title 15A Subchapter 8G Section .0201 requires an owner of a classified water pollution control system to submit a signed completed"Water Pollution Control System Operator Designation Form"to the Commission countersigned by the Town of Kenly,WQCS00173 June 22, 2018 designated certified operators within 120 calendar days following a vacancy in the position of ORC or Backup ORC. In the Division's data system, the designated ORC for the Town was Kenneth Thompson. In July 2017, Mr. Thompson discontinued his employment with the Town. Please complete the attached WPCS Operator Designation Form and submit it to the Division. 12. Cleaning Sewer Lines. The Town has developed and implements a program for cleaning the sewer lines. As required by Section II, item 8 of the subject permit, at least 10% of the wastewater collection system should be cleaned each year, which is a minimum of 8,184 feet for the Town's system. Preventative cleaning is not required for sewer lines less than five years old, unless an inspection reveals the need for cleaning. As the Public Works Director indicated during the inspection, 3470 feet of sewer line was cleaned during calendar year 2017, and 320 feet of gravity line and 1100 feet of force main were replaced during the spring of 2017. Please provide this office with the Town's plan to develop and implement a program for appropriately cleaning all sewer lines, including equipment that will be used. Please specify if the Town owns or will rent the needed equipment. 13. Spill Response Action Plan. The Town has developed a Spill Response Action Plan that includes 24- hour contact numbers, response time, equipment list and spare parts inventory, access to cleaning equipment, access to construction crews, sources of emergency funds, site sanitation and cleanup materials, and post-SSO assessment. The Town should periodically update the Plan to reflect any changes in business process or personnel changes. The inspectors observed Mr. Kenneth Thompson is still listed in the Plan as a contact. 14. Comprehensive Sewer Map. Section 3, item 3 of the subject permit requires the permittee to maintain a comprehensive map of its wastewater collection system. At a minimum,the map should include pipe size,pipe material, pipe location, flow direction, approximate pipe age, number of active service taps, and each pump station identification, location and capacity. During the inspection, John Pitts stated the Town has a map, but it was not available for the inspectors' review. Within thirty days of receiving this letter,please provide this office with a copy of the Town's current wastewater collection system map. 15. Inspection of High Priority Lines. As required by Section V, item 4 of the subject permit, an inspection of all high priority sewer lines shall be performed at least once every six-months. The Town inspects the high priority lines twice per year in June and December. The inspectors observed log entries that showed the last high priority line inspections were conducted on December 5, 2017, and June 5, 2018. During the state inspection, the following high priority line was inspected: the suspended 8-inch line at the Town's wastewater treatment plant. No compliance issues with the line were observed. The Town recently discovered two additional high priority lines off Truck Stop Road across from the Town's wastewater treatment plant. These 8-inch aerial lines appear to be made of ductile iron with mechanical joints. The pipelines appeared to be sufficiently supported and restrained and free of evidence of leakage and visible damage. This office will recommend that these pipelines be included in the reissued permit as high priority lines. Photographs of the pipelines are included as Attachment A.
