Corey Basinger: NEWTON NORTH CAROLINA RECEIVEI)MCDENR/DWR JUN 21 2018 WOROS MOORESVILLE REGIONAL OFFICE June 15, 2018 In regards to Notice of Violation NOV-2018-MV-0103 during the month of January 2018 we were going through major personnel changes in our Lab and Pretreatment Departments. We had just named a new Pretreatment Coordinator and hired a new Lab Analyst to replace her in the lab. With the majority of our lab personnel being newer and not knowing what we did on a monthly basis, putting out the acid washed jugs was overlooked in January. We had a disgruntled employee who supposedly "Forgot" as well. In trying to train and help the new Pretreatment Coordinator, I forgot as well. The mistake is on me as I should have been doing a better job and checked better. It was caught by the Pretreatment Coordinator in the early part of February. We checked to see if we had any samples that we could use, we didn't. I called Wes Bell and advised him of the mistake in not getting the samples. Going forward we have made several changes to insure that this doesn't happen again. The Pretreatment Coordinator is now the crew leader in the lab and one thing she has done is write on the calendar the days that we will do the acid washed jugs each month. We have also trained all lab personnel to check each month to make sure that we have done them. We are making sure that they are trained in all stations in the lab in the event we have one out, they will know what and where to check. They are also trained in getting the samples and chain -of -custodies ready for the commercial lab to pick them up. They are also double checking behind each other to make sure all samples are gotten for the month. We are asking for leniency with this violation due to the circumstances that this happened, this has never happened and with the changes that we have made, we don't feel like it will happen again.
