Purvis 12604 Bell Stone Lane RECE9l/E®/®ENR/DWR Raleigh, NC 27614 JUN 2 6 2018 Subject: Compliance Evaluation Inspection Water Resources Single Family Wastewater Treatment System Permitting Section Permit No. NCG551270 515 Oak Pointe Dr. Person County Dear Permittee, On June 13, 2018, Rick Trone from the Raleigh Regional Office attempted to inspect your single-family residence (SFR) wastewater treatment system to evaluate compliance with the above permit to discharge wastewater. Staff did not have a valid phone number or access to the home and therefore could not complete the inspection. The checked boxes below show what conditions were noted at your facility: ® NPDES Permit Name/Owner Change Form: As the new owner of 515 Oak Pointe Drive according to Person County record of deeds), you are also the new owner of the existing single-family wastewater treatment system, which treats the domestic wastewater from your residence and releases the effluent to receiving waters known as Hyco Lake. Because your treatment system makes an outlet to waters of the state, it is an activity for which the subj ect permit is required. To comply with North Carolina General Statute § 143-215.1(a), which requires a person to obtain a permit to make an outlet into the waters of the state, you will need to complete and submit the attached NPDES Permit Name/Ownership Change Form to the Division within 45-days receipt of this letter. ❑ In compliance: You are reminded to regularly maintain the chlorine disinfection and dechlorination systems, have the effluent sampled once a year, and have the septic tank pumped out every 3 to 5 years. Your good record of operation and meeting the permit requirements is highly commended. ❑ Disinfection: Your system is lacking disinfection, either chlorine tablets or a UV light system. New rules put into place on August 1, 2007 require all SFR systems to have a means of disinfection (and dechlorination when chlorine tablets are used to disinfect, if the system was installed since that date). Since your system had no disinfection, the installation is to include a chlorine tablet dispenser, a contact chamber capable of providing a minimum 30 minute contact time, and another tablet dispenser that will hold dechlorination tablets. Please submit a schedule to this office within 20 calendar days of receipt of this letter that states your plan for correcting this deficiency. ❑ Treatment tablets missing or are wrong Bind: You are responsible for always having chlorine tablets and dechlorination tablets (if a required part of your system) in place. They must be the kind for wastewater treatment and not for swimming pools. See Disinfection paragraph above. ❑ Pumping the septic tank: The septic tank should be pumped out every 3 to 5 years. A pumping company can check the status periodically and determine when pumping is required. F] Failure to analyze the effluent: The effluent that is discharged from your system must be analyzed once each year. See Part I(A) of your permit about this requirement. A list of NC certified laboratories that provide this service was left at your residence during the inspection. ❑ Locations of treatment units are unknown: Determine this and report to this office within 30 days of receipt of this letter with a sketch or map.
