Answer questions for Parts A through C by checicing either "Yes" or °No". Complete Part D of the checklist when Miniinum Criteria Rule categories #8, #12(i), or #15 are used. TIP Project No.: n/a State Project No.: 17BP.14.R.111 Project Location: Bridge No. 220 on SR 1351 (Matlock Creelc Road) over Matlock Creek Macon County, North Carolina Project Description: Replace Bridge No. 220 with 45' 6" L x 16' 2" W x 5' 1" H aluminum box culvert on existing location, near Franlclin, North Carolina. The project will utilize an on-site detour during construction. Anticipated Permit or Consultation Requirements: Nationwide Permit No. 14 and 401 Certification. Written notification is required for this project because it is a bridge to bottomed culvert conversion. 1. Outside river corridor = Yes, (greater than 0.50 iniles from Cowee Creelc & greater than 0.75 iniles from Little Tennessee River) 2. Below iinpact threshold for applicable NWP = No, bridge to culvert (mitigation required). NWP 14 Required. 3. Tree clearing = Yes (poplar, maple) —(follow moratorium). 4. Trout watershed = Yes (follow moratorium unless verification NCWRC letter states no). 5. NLEB HUC = Yes, greater than 025 miles from known hibernacula site. NWP 14 will require written authorization and written 401 Certification. Special Project Information: A listed species evaluation and cultural resource screening information are included in project file at Division 14 Environmental Office. The bridge and surrounding area were surveyed on 6.25.13, 5.30.14 and 5.14.2018 and no listed species were observed. Suitable habitat for listed species known from Macon County appears lacking in the project area. There are records for Appalachian Elktoe mussels and Spotfin Chub in the Little Tennessee River, respectively, but occupied habitat is over 3.12 miles downstream of bridge 220. There are no records for listed species records within 1 mile of the bridge and the bridge is too low to support bat roosting (lack of bat roosting will be confirmed before construction). Tree clearing will occur from October I S to April I S to protect possible Indiana bat roosting. 06/07/18 1 of4 PART A: MINIMUM CRITERIA Item 1 to be completed by the Engineer. 1. Is the proposed project listed as a type and class of activity allowed under the Minirnuin Criteria Rule in which environmental docuinentation is not required? If the answer to number 1 is "nd', then the project does not qualify as a ininimum criteria project. A state environmental assessment is required. If yes, under which category? (9) Reconstruction of existine crossroad or raitroad separations and existin� stream crossinss, includine, but not limited to pipes, culverts, and bridEes. If either category #8, #12(i) or #15 is used complete Part D of this checklist. PART B: MINIMUM CRITERIA EXCEPTIONS Itenas 2— 4 to be completed by the Engineer. 2. Could the proposed activity cause significant changes in land use concentrations that would be expected to create adverse air quality impacts? 3, Will the proposed activity have secondary impacts or cumulative impacts that may result in a significant adverse impact to human health or the environment? 4. Is the proposed activity of such an unusual nature or does the proposed activity have such widespread implications, that an uncoinmon concern for its environmental effects has been expressed to the Departinent? A trout moratorium may be applied to the permits at the request of WRC. • Project will adhere to NCDOT best management practices. Avoidance of erosion and sedimentation into Matlock Creek and downstream habitats. • Trees will be cleared from October 15 to April 15 to avoid potential roosting by Indiana bats.
