Please accept this letter as the written follow-up to the notification made to Tim Latham, Sue
Homewood and Bryan Roden -Reynolds on June 13, 2018 at 7:51 am regarding the Piedmont
Natural Gas Clemmons Area Upgrade Pipeline project in Davie County, North Carolina.
On Tuesday, June 13, 2018, the Environmental Field Support was notified about erosion and
sediment control releases following an intense rain event that occurred the previous evening.
Despite the addition of extra erosion and sediment control BMPs the previous week, there were
multiple sediment releases as follows:
Station 55+82 — sediment release to stream
Station 89+54 — sediment release to stream/wetland
Station 168+00 — sediment release to stream
Station 187+24 — sediment release to stream
Station 201+03 — sediment release to wetland
Station 215+03 — sediment release to stream (rock/flume crossing)
Station 293+80 — sediment release to wetland
Upon discovery of the releases crews immediately focused on removing deposited sediment
from the above locations by hand. The intense rain event from 6/11/18 overwhelmed areas of
silt fence and these areas were repaired, additional silt fence was added, wattles were replaced
and slope breakers were repaired. Temporary stabilization measures (seeding) have been
taken in select locations. The project team is also evaluating areas where additional slope
breakers can be added and they are in the process of installing silt fence outlets in critical
locations per Tim Latham's guidance from the 6/13/18 inspection of the project site.
