I have reviewed the information and need clarification before proceeding with verifying the after -the -fact use of a Nationwide Permit. Please submit the requested information below (via e-mail is fine) within 30 days of receipt of this Notification: 1) On the PCN, please request to use Nationwide Permit 18; 2) On the PCN, part D.3h, please revise the Impact Length. This should be the stream impact of the culvert only. As such, please use a tape measure to measure the culvert length from end-to-end, and then multiply this amount by 1.2 to account for the presumed sinuosity of the stream prior to the culvert being installed. The resulting Impact Length should be considerably less than 144 linear feet; 3) Please revise and resubmit the Plan View and Profile View to show the culvert as currently installed. The Plan View and Profile View Example shows two culvert pipes 60" in diameter, whereas the culvert installed for your driveway is only a single pipe and much smaller in diameter (18"?). As an alternative, you could take a picture of the culvert from the wetland looking downstream, and describe the culvert length and diameter. 4) Note also that the aerial photo shown below the Plan View and Profile View Example does not show your property. Here you can refer instead to the Unauthorized Activity Sketch sent with my 5/16/2018 letter. Please let me know if you have any questions. -Dave Bailey David E.
