Dear Ms. Woodard:
The Division has reviewed your application, received on November 14, 2017, for a new NPDES
permit NC0089737. To enable us to complete our review in accordance with N.C.G.S. 143-215.1
and 15A NCAC 2H .0105, we need additional or revised information regarding your discharge:
1. The schematic provided with the application should show the proposed sampling point, the
outfall, and the flow of the discharge through the property all the way to the discharge point.
The updated information received by the Division is unclear as to where the proposed
sampling point will be located. Please indicate where the proposed sampling point will be
along with the outfall location.
2. As stated in the previous additional information request, all new applicants are required to
provide an analysis of the source water. The information provided to the Division is
incomplete. As this is an existing water treatment plant, a complete analysis of the proposed
discharge is required for the following parameters: Aluminum,Arsenic, Barium, Beta
Particles, Calcium, Chloride, Copper, Iron, Magnesium, Manganese, Ammonia-Nitrogen,
Nitrogen-Nitrate+Nitrite, Total Phosphorus, Total Nitrogen [TN=TKN+ (Nitrites
+Nitrates)], Total Dissolved Solids, Total Suspended Solids, Zinc, Lead,pH, Radium 226 &
228 (pCi/L), and three (3) aquatic toxicity tests using Ceriodaphnia dubia at 90% effluent
concentration, or estimated dilution, or perform a multi-concentration test. The results for all
samples tested should be submitted to the Division for review. If you have questions regarding
the appropriate test methods for the toxicity tests, please contact Susan Meadows with the

3. The previous additional information request asked for information from US Geological
Survey staff regarding 7Q10, 30Q2, and Qaverage at the location of Outfall 001, located at

4. Step 3 of the EAA requires alternatives be evaluated. Two of the alternatives required by the
EAA to be evaluated are connecting to an existing WWTP (Alternative A) and land
application(Alternative B). Materials provided have stated that connecting to an existing
WWTP (Alternative A) is not a feasible alternative. Alternative A(1)(b) states: "Provide a
preliminary indication of flow acceptance from existing municipal or private WWTPs under
consideration for connection. If a municipal or private WWTP cannot accept the wastewater,
include a letter documenting such and consider this alternative technologically infeasible."
This letter has not been submitted to justify an existing WWTP not accepting the wastewater.
Alternative B requires that a subsurface system(septic drain field) and drip irrigation be
evaluated. These alternatives were not evaluated. Please submit the required information
for Alternatives A and B.
If no response is received within 60 calendar days [per 15A NCAC 2H.0107(b)], the application
