The project involves moving the stream back to its original flow path, away from the manhole and sanitary sewer
line at two locations.
4g. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
Speight Branch has been eroding significantly in the past year and is threatening to wash out a manhole and
expose the sanitary sewer line at two locations within the work area. The project entails placing fill and excavating
accumulated sediment to return the stream to a more stable flow path away from the utility infrastructure. The
restored banks will be armored with riprap to prevent the erosion from threatening the infrastructure in the future.
This is an emergency repair project as the manhole is in danger of being washed out and PVC sewer lines
exposed.

6i. Comments:
The purpose for the project is streambank stabilization and protection of eAsting infrastructure. Access through
buffer will be achieved along eAsting sanitary sewer easement. Impacts associated with the proposed project will
include placement of fill to move stream channel back to its original configuration. The permanent buffer impact is
fill and riprap in the old channel locations, creating new streambanks with 2:1 slope. Riprap will be used to armor
the banks to prevent future erosion. Several trees at the edge of the streambank to be stabilized will be cut to
facilitate laying the bank back.

1a. Specificallydescribe measures taken to avoid or minimize the proposed impacts in designing the project:*
Work is limited to two areas where the stream is in close propmity to the sanitary sewer line. Access to the work
area is via etasting sanitary sewer easements. The project will be completed during a relatively low -flow period.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:*
Disturbance will be limited to the two work areas in order to return the stream to its original flow path. Any ground
disturbance due to access will be graded to original contours, strawed and seeded. A pump -around will be
employed and work will be done "in the dry".
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
