Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* Approximately 40% of the lot is mature forest of steeper terrain. The remaining property is open, most of which is comprised of tobacco production fields. The impacted stream is a perennial, spring -fed creek running north -south on the west side of the property. Explain the purpose of the proposed project:* Install a culverted road crossing to the owner's proposed home site. No other access options are available to get to the home site, and no feasible building site locations are located between the public roadway and the stream. 4g. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* Prior to construction, the owner and contractor will demarcate LOD and tree protection zones. Contractor will mobilize equipment at SE property corner off of Williard Rd. and install temporary construction entrance, followed by tree removal, grubbing, rough grading on proposed road on west side of stream. Subsequent steps include installation of the temporary dikes, commencement of the stream pump -around, and installation of the culvert/headwall. During initial bedding and backfill, the contractor will be able to access the east side of the creek and begin tree removal, grubbing, and rough grading within the proposed LOD. Following finish grading, contractor will install 6" road gravel and rip rap armor within stream corridor. Ditch lines will be graded with maximium slope of 3:1 and stabilized with excelsior matting and intermittent rock checks. A tub grinder will be used on site to chip woody debris for site stabilization. Large logs will be cut to length and stockpiled onsite. Additional trees will be selectively cut outside of the LOD for sawing onsite. Primary equipment to used will be a Deere 350G excavator, Deere 700 FV750 K dozer, and CAT (or equivalent) 48"/54" padfoot roller for grading and culvert installation. 4h. Impact Justification and Mitigation 1. Avoidance and Minimization 1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:* Bridge option was initially evaluated but determined to be cost prohibitive. For the culvert, endwall structures and rip rap slope armoring are proposed to minimize pipe length. An over -sized 48" RCP is proposed to also minimize fill and improve aquatic wildlife passage throughout the impacted segment. Tree removal and limits of disturbance are minimized to the extent practical to reduce impact to both the riparian and forest systems. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:* Al work in the stream shall be performed in the dry by using temporary dikes, pump -around, and dewatering system (silt bag) as needed.
