During the inspection, a review was conducted for NC -RA -01; a 10.9 -acre field owned by Mr. Guy Byrd. The field appeared to be properly buffered. No GW monitoring is required for this site. Application equipment calibration is to be submitted to the MRO office. Page: 3 Permit: W00011431 Owner • Facility: Town of Cleveland Inspection Date: 06/11/2018 Inspection Type : Technical Assistance Reason for Visit: Routine Inspection Summary: On Monday, June 11, 2018, MRO staff met with Ray Gambit of Bio -Green to review Town of Cleveland Residual Land Application (RLAP) field NC -RA -01-01. The buffer areas covered in the permit have been observed and are inclusive of the onsite drainage feature. An additional area has been pushed clear at the request of Mr. Guy Byrd who owns the adjacent property to the land application field. This clearing does not interfer with the buffers for the field as it is along a slope greater than 10% and application is prohibited in this area of the field. An extra signed was requested by MRO to be placed at the enterance from Mr. Byrd's property as public notification of the land application event. No land application was observed during the site visit. Equipment calibration documention is to be provided to the MRO office. Page: 2
