CORRECTION to Transmittal Letter

Dear Mr. Hoffman:
In accordance with your application for a discharge permit received on February 5, 2018, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
This COC was issued on April 24, 2018; however, the permit package was inadvertently sent to
the wrong address. We just received the misdirected package back in our office on June 13, 2018.
We apologize for this error and long delay in getting these materials to you.
Please note that the NCG210000 General Permit will expire on July 31, 2018. Typically all permit
COCs are renewed automatically once the General Permit itself is finalized, unless there are
outstanding annual fees. There is no need to reapply for a renewal unless the Division directs you
to do so. Be sure to check our program website for updates and public notice of proposed renewal
actions in 2018.
Please take notice that this certificate of coverage is not transferable except upon approval of the
Division of Energy, Mineral, and Land Resources. The Division of Energy, Mineral, and Land
Resources may require modification or revocation and reissuance of the certificate of coverage.

This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Resources, the Coastal Area Management Act, or any other federal or local
government permit that may be required.
