REGAN Secretary LINDA CULPEPPER Interim Director Subject: Attachment B Certification Permit No. WQ0000702 City of Havelock RLAP Craven County In accordance with your permit modification request received May 17, 2018, we are forwarding herewith a modified Attachment B certified June 13, 2018, to the City of Havelock for the subject residuals program. This certification is being issued to modify Attachment B by addition of nine additional fields in Pamlico County totaling 736.6 acres, those fields being NC -PA -03-01 to -09. This modified Attachment B shall be effective from the date of certification until December 31, 2021 (i.e., expiration date of the current permit), shall void Attachment B of Permit No. WQ0000702 issued January 17, 2017, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace the Attachment B of Permit No. WQ0000702 that was issued January 17, 2017 with this modified Attachment B. Please note that only the maps for new land application sites are attached to this letter. It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements, and/or limitations contained in this certification or modified Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
