Dear Mr. Vance:

The self-reported Bypass 5-Day Report submitted by Water Quality Labs indicates violations of permit
conditions stipulated in the subject permit. Violations include an unauthorized bypass from the wastewater
treatment facility to the land or surface waters that is prohibited in Permit No. NC0026654 Part II.C(4)c(1):
Bypass from the treatment facility is prohibited and the Permit Issuing Authority may take enforcement action
against a Permittee for bypass.

Specific incident(s) cited in the subject report include the following:
A 150 gallon unauthorized bypass from the equalization basin of the Crossnore WWTP
occurred on April 24, 2018.

Remedial actions, if not already implemented, should be taken to correct the above noncompliance.
