Dear Sampson's Eggs, Inc:
This Notice of Deficiency is being sent to you for failure to submit the 2017 Annual Certification
Report. One of the requirements of your Animal Waste Management NPDES General Permit is
the submittal of an Annual Certification Report and Sludge Survey result to the Division of
Water Resources by March ] at of each year. As of this date, our records indicate that the 2017
annual certification report and sludge survey result has not been submitted for the subject
facility.
Failure to act in accordance with the terms, conditions, or requirements of your permit may result
in a recommendation of enforcement action, in accordance with North Carolina General Statute
§143-215.6A.
Please respond to this Notice of Deficiency within 15 calendar days of the receipt of this letter by
submitting the Annual Certification Report and Sludge Survey result for the year 2017 from your
facility to the following address:

Nothing in this letter should be taken as relieving from you the responsibility for failing to
