The assistance and help provided by the City of Henderson personnel and collection system operators with this inspection was greatly appreciated. Below is a list of findings developed from the inspection and review of the permit: 1. The permit was reissued and became effective May 01, 2018 and will expire April 30, 2026. There were no significant changes made to the permit. 2. The City of Henderson operates and maintains a Class-3 collection system. Basinwide Information Management Systems (BIMS) lists Joey Long as the CS grade-4 Operator in Charge (ORC) and Andrew Perkinson as the CS grade-2 back-up ORC. 3. A map of the collection system is maintained however it does not include line size, flow direction, pump station location, and the type of pipe material. A GIS map is being developed with all map requirements. The GIS map is 25% complete. 4. The Capital Improvement Program was updated this year and includes projects for fiscal years 2018 through 2024. Known deficiencies have been identified and are prioritized by importance. Some projects include upgrades to Sandy Creek and Red Bud pump stations, upgrades to the water reclamation facility, and changes to the Chicken Farm pump station to a gravity system. 5. Education materials on proper grease disposal are sent to customers twice a year. Information on proper grease disposal is also available on the web. Food establishments are inspected throughout the year. There have been no enforcements for the past year. 6. Work orders, complaints, repairs made to equipment, line cleaning, right of way maintenance, high priority line inspection, SSO's, pump station maintenance are maintained in separate log books. According to the permit, the amount of gravity sewer in the collection system is 147 miles or 776,160 feet. A review of the records indicate that 102,000 feet have been cleaned this year. 8. BIMS lists seventeen Sanitary Sewer Overflows (SSO's)for the review period May 2016- May 10, 2018. Documentation of the SSO's and SSO's that are below reporting thresholds are being kept. 9. There are thirteen pump stations in the collection system. All pump stations have restricted access, contact information signage, and audible / visual alarms. Red Bud and Sandy Creek pump stations are connected to SCADA. The other eleven pump stations are connected to telemetry. There are seven pump stations with on-site generators, with one portable generator, three pumps, and one 40,000 KW generator available for emergencies. Pump stations are inspected three times a week. 10. Pump stations inspected were Wortham Ct., Martins Creek, Industrial Park, and Zeb Robinson. There were on-site generators at Wortham Ct., Martins Creek, and Industrial Park. The visual alarm did not function at Wortham Ct. when the high water was lifted. All other audible / visual alarms function when the high-water float was lifted. All on-site generators operated under load when power was switch off to the pump station. Telemetry phoned in a high wet well and power outage. The wet well at Zeb Robinson had an excessive amount of grease which should be cleaned. All other wet wells appeared normal. 11. The Old City Hall and Hughes Outfalls were inspected. Both outfalls have aerial creek crossings. All concrete piers appeared solid with no undermining. Both aerial lines appeared straight with no bows. Flow in manholes was free from obstruction. The right- of-ways were well maintained with no sinkholes or depressions. Within thirty days receipt of this letter, please respond to when the visual alarm at Wortham Ct. will be repaired and when the Zeb Robinson wet well will be cleaned of excessive grease.
