Attach a vicinity map (i.e. street map) clearly showing the location of the property with respect to local landmarks such as towns, rivers, and roads. 2. You may use the diagram provided at the end of this application form. 3. Attach a photograph of the shoreline/ buffer proposed to be stabilized. (Include a scale of some sort- a yard stick, shovel handle, etc.) 4. Pass the horse stables and take a quick sharp right on Valwood. Take the second left onto Wedgewood Drive. Take your first right onto Dorsett Lane. Describe the existing land use or condition of the site at the time of this application (residential, undeveloped, etc.): Residential 6. Property size (acres): .92 acres, 7 Lake/ river/ ocean adjacent to property: Lake Hickory, 8. Describe proposed work (include discussion as to how hardening of shoreline has been avoided, or why it is necessary): Currently our home/structure is being compromised by the lake's natural erosion and is dangerously close to destruction in its increasingly close proximity to the shoreline. We need to implement lake erosion control and secure our home and safety of family and guests. We are requesting a permit to install rip rap along our shoreline. Tree removal will need to be part of this project, as equipment must be able to safely enter the area to be treated. Given how little earth there is between our home/structure and the water, we are requesting minimal requirements in the degree of shoreline grading, in order to protect and maintain as much distance between the home and the lake as possible. We have engaged with Bruce Black to do the work described. He has met with Wade Harmon onsite on 5/1/18 regarding the project to ensure we are following all rules and regulations of Duke Energy in completing the project. Total amount of disturbance (including all clearing, back fill, excavation, rip rap, retaining walls, etc.) above the normal pool lake level/ Normal Water Level and 50 feet land -ward in square feet or acres: Removal of up to 11 trees, maybe less, as shown on the diagram. Only trees that need to be removed for equipment access to the treated site will be removed and we will try to take less than the 11 if possible. Shoreline will be minimally tapered in order to hold the rip rap for erosion control. 12. Please describe the vegetation above the normal pool lake level/ Normal Water Level and 50 feet landward to be impacted (number of trees, for instance): Removal of up to 11 trees as mentioned above and shown on the diagram.
