Because the original COC showed the correct waterbody and this was our mistake with connecting the wrong segment in
our BIMS database to your permit, I think we can handle this without the process of a formal COC modification. With
the information you provided (original COC, outfall locations, and mine site plan), I have made the revision in
BIMS. Attached is a report to verify the correction for NCG020255. Please review it to make sure it's accurate (I
swapped the coordinates for the outfalls as you advised).
If it's acceptable to Martin Marietta, simply keep this report with the permit records to inform your monitoring
requirements for this site and document the correction. The next renewal COC (scheduled for 2020) should reflect the
right waterbody and classification.

The original NOI is not in the folder, but the original COC is there. Like your copy, the original
NCG020255 COC shows the New River as the receiving water, and this is consistent with the outfall
locations on a map. Evidently, the wrong segment (index number #19-37-1, much farther downstream
beyond Stone's Bay) was associated with this permit COC in our BIMS database, instead of index #19-(1),
which denotes the segment of the New River in the vicinity of this mine location.
Once it's corrected in BIMS, future renewal COCs will show the New River with classification C;NSW for
this discharge.
I can add both these outfalls into BIMS and make sure they have the right waterbody. What type of
water is discharged from each outfall — Wastewater (mine dewatering, etc.), Stormwater, or both? Once
I make the correction, I'll send you verification for your records, and it can be kept with the COC and
other permit records. Future renewals should be correct.

Thanks for the quick response. Attached is a copy of the original permit. The original NOI did not have
physical location information for the outfalls, just the number. Below are coordinates of the two pit
discharge outfalls at Onslow Quarry. (If you need a physical address: 131 Duffy Field Road, Richlands,
NC.)
Outfall 001— 34.841989 ON -77.543631°W
Outfall 002- 34.832644 ON -77.543992 °W
It seems like the only issue may be that the unnamed tributary came to be associated with Rufus Creek,
which is a HQW. Please let me know the best way to proceed. If there is an error, we want to make
sure to fix it.

Can you give me a lat/Ion for the outfall in question?
We should verify whether the original NOI was submitted with an error about the receiving waterbody
(if it's available -- it may not be in our files but in archives now. It would be helpful if you have a copy of
this application paperwork on hand). If this was the case, MM needs to submit a
correction/modification request because the NOI information initially submitted to DENR/DEQwas not
accurate.
If we cannot access the original NOI, we will still need to modify the COC after confirming the waterbody
on the COC is not accurate. If you can provide us (1) a coordinate or map of the outfall location and (2) a
current mine site plan showing the layout with the outfalls (SW and WW identified), we can confirm the
change is necessary and proceed with a modification.
I will check the file folder for the original NOI on Monday.

Hi Bethany,
I hope you are well. I was reviewing several of our discharge points and came across something a bit
odd. Our Onslow Quarry (COC 20255) located at 131 Duffy Field Road in Richlands, NC discharges to
Rufus Creek according to COC. When looking at the stream classification map, Rufus Creek appears to
be somewhere else. We just wanted to check to see your interpretation of our discharge points. It
looks to be an unnamed tributary that discharges into part of the New River classified as C, which would
not require the HOW sampling. What do we need to do to alter the COC to remove the HOW discharge
designation?
