WBS No. 38583.1.1, TIP No. B-4813 Dear Mr. Shaver and Ms. Steenhuis: The North Carolina Department of Transportation (NCDOT) is proposing to replace Bridge No. 8 over Crane Creek on SR 11004 (Edmond Matthis Rd.) in Sampson County. The existing 4 span - 69 ft. bridge will be replaced with 2 span - 105 ft. bridge. A preliminary jurisdictional determination (PJD) was acquired on this project on July 3, 2017 and the project was assigned Action Id No. SAW-2015-01089. Impacts associated with the project include 0.06 acres or permanent wetland impacts for fill, excavation and mechanized clearing and 41 lf of bank stabilization at End Bent 2. Hydraulics’ ran velocity calculation along this bank, which was on the outside of a bend, and velocities were greater than 2 ft. /sec. The attached permit drawing provide more details on the proposed impacts. Regulatory Approvals Section 404 Permit: NCDOT is proposing to construct this project under the conditions of Nationwide 3 & 13. Based on the current design and impacts, notification requirements are not applicable for this project and written approval from the USACE is not required. NCDOT will abide by all conditions listed in NW 3 & 13, and will include a copy of the certification in the permit package and/or contract. Section 401 Permit: The associated 401 General Certification 4132 and 4134 will also apply to this project. Notification thresholds or circumstances for these certifications are not applicable to this project therefore written concurrence from the Division of Water Resources (DWR) is not required. NCDOT will abide by all conditions listed in GC 4132 and 4134, and will include a copy of the certification in the permit package and/or contract. The runoff from the deck drains will be discharged onto a class B rip rap dissipator pad. Additional measures implemented to reduce impacts to the stream and surrounding properties include, maintaining existing drainge patterns, using special lateral 'V' ditches.
