Dear Thomasine Dade,
On May 1, 2018, staff of the NC Division of Water Resources (DWR), Water Quality Regional
Operations Section (WQROS), inspected the subject single family residence surface irrigation
wastewater treatment and disposal system. I would like to thank you and Mr. Moore for your
assistance during this inspection. The purpose of the visit was to conduct a compliance inspection
and complete the requested name change.
On the day of the inspection all single family residence wastewater treatment and application
equipment appeared to be well maintained. Floats were not checked because of the depth to float.
System was operated manually and appeared to be operating properly. The spray field (shared
with Mr. Moore)was in good condition and all heads were operating properly.

On May 10,2018, staff of the NC Division of Water Resources,conducted a compliance inspection for the
subject single-family irrigation system. We appreciate Mr. Carroll's assistance during the inspection.
Attached is a copy of the inspection report for your records.
On the day of the inspection, the wastewater treatment units and irrigation equipment appeared to be
adequately maintained. The irrigation area was in good shape and there was no ponding of water. The
irrigation also had a well-maintained fence.
We discussed setbacks from the neighboring property. Your permit requires a 150-foot setback from the
wetted area to neighboring property lines. Accordmg to the engineering plans from 1992,Mr. Morris had
granted a setback waiver to your father(Mr.J.L. Carroll). However,there doesn't appear to be any record
of this waiver. Additionally, it appears that the property line to the west of the irrigation area (owned by
your father) is not 150 feet away. Your permit expires November 30, 2018 and is due for renewal.During
the renewal process of this permit, getting setback waivers from both properties might be required so that
future issues do not arise regarding required setbacks.
