4. Project Description
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The project will rehabilitate a major sewer outfall of 30 inch reinforced concrete pipe which was installed in 1973.
The pipeline was installed along Briery Run an Stonyton Creek, and it is in the floodplain of these two main
drainage basins in Lenoir County. The soils and physical presence of water and wetland plants classify the area
as wetlands. The plan is to use "no dig" methods wherever possible, and access the sewer line and manholes
using construction mats

4e. List the total estimated linear feet of all existing streams on the property:
(intermittent and perennial)
Stream Length 13,131 feet, Property, Easement and Wetland Information.
4f. Explain the purpose of the proposed project:*
The purpose of the project is to rehabilitate a major Sewer outfall installed in 1973. The concrete pipe is
deteriorated and experiencing significant inflow and infiltration. The project is designed to add life to the pipe which
is approaching its life expectancy of 50 years, and to reduce the inflow and infiltration.
4g. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:
Project will consist of laying out construction mats to provide access for equipment such as Track hoes, Trucks,
Cleaning and TV Equipment, Equipment for installing Cured in Place Pipe, Bypass Pump equipment, and other
equipment as needed. Access will be provided by using construction mats which will minimize the impact to
wetlands, streams and buffers. The thirty inch concrete sewer pipe will be rehabilitated using no dig methods,
cured in place pipe. All manholes will be raised to two feet above the 100 year floodplain, and manholes will be
rehabilitated using a cured in place liner.

1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:*
Construction Mats will be used for access to the construction site. The construction mats will minimize damage to
plant and animal life and to the terrain caused by moving equipment to each manhole and section of pipe.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:*
Wherever possible, "no dig" repair or rehabilitation will be used, and construction mats will be used to provide
access to each manhole and/or point repair.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
r Yes
C No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:
The project temporary impacts will not result in any temporary change in the eAsting plant or animal life or the contours of the eAsting
terrain. Stormwater will continue to flow along the same path after construction as before construction.
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
*** Recent changes to the stormwater rules have required updates to this section .***
1. Diffuse Flow Plan
1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection
Rules?
C Yes
No MR
1b. All buffer impacts and high ground impacts require diffuse flow or other form of stormwater treatment. If the project is subject to a
state implemented riparian buffer protection program, include a plan that fully documents how diffuse flow will be maintained.
All Stormwater Control Measures (SCM)s must be designed in accordance with the NC Stormwater Design Manual. Associated
supplement forms and other documentation shall be provided.
What type of SCM are you providing?
❑ Level Spreader
❑ Vegetated Conveyance (lower SHWT)
r Wetland Swale (higher SHWT)
r Other SCM that removes minimum 30% nitrogen
(check all that apply)
