Holman St. NPDES Permit NCG510444 Durham County Dear Mr. Getty, On May 02, 2018, Mitch Hayes of the Raleigh Regional Office conducted a Compliance Evaluation Inspection at the subject facility. Your assistance with this inspection was very helpful. Below is a list of findings made during the inspection and review of the permit: 1. The existing subject facility consists of the following units: six recovery wells each with six gallons per minute (GPM) pump, a 36 GPM oil / water separator, gravity flow to a 250-gallon free product tank, a 36 GPM low profile air stripper, four particulate filters in parallel and series, two carbon vessels in series. 2. The groundwater remediation facility was shut down at the time of inspection. Electrical power had been disconnected. The building was locked. According Division of Water Resources (DWR) records, the facility has not operated since 2005. 3. According to the Underground Storage Tanks (UST) Section, groundwater incident number 4006 is not closed out. Mark Powers, Supervisor of the UST section, stated that the current groundwater remediation system on site could be removed however, the monitoring wells must remain intact. Future groundwater remediation may require the use of a non-discharge groundwater remediation system. Please be aware that your permit requires the system to be properly operated and maintained at all times by the permittee. Please explain how you intend to accomplish this if you wish to continue holding this permit.Otherwise,you may request rescission of this permit. 4. If you wish to request rescission of this permit, please Contact Charles Weaver at charles.weaver(c�ncdenr.gov or 919-807-6391. 5. Please respond to above items 3 and 4 in writing within 30 days of receipt of this letter.
