On behalf of the Town of Burnsville please find enclosed one (1) original and one (1)
copy of the following for the above referenced project:
• Executed Fast Track application form FTA 04-16
• Check in the amount of $480 for the application fee
• An 8.5 by 114nch portion of a llSGS map showing the project area
• A street/location map showing the project area
The project consists of the installation of approximately 2,000 linear feet of 8 -inch
sanitary sewer, 3 900 linear feet of 4 -inch sewer service line, and m o es. a pr sect will
replace the existing 4 -inch Orangeburg sewer lines current y located within the Peterson Trailer
Park with sanitary sewer lines sized and constructed of materials conforming to current approved
standards for public sewer construction. The new sewer lines will connect to an existing manhole
at the intersection of David Park Drive and SR 1374.
The Town of Burnsville will own and maintain the gravity sewer lines. No additional
flow will be generated with this project. We have not included Flow Tracking/Acceptance Form
FTSE 04-16 due to the fact that the project does not generate any additional wastewater.
