EXPRESS REVIEW ACCEPTANCE LETTER

Subject Property: Jamestown Middle School (SMP)
On May 13, 2009, the Division of Water Quality (DWQ) received a request from Mr. Steven Miller of
CLH Design, PA for acceptance into the Express Review Program for a review and approval of the
Stormwater Management Plan (SMP) for the subject project in Guildford County. This letter advises you
that your project will be accepted into the Express Review Program once the following items are
received:
a. The application fee of $1500.00 made payable to the North Carolina Division of Water Quality
[Stormwater Management Plan (SMP) with four (4) drainage areas];
b. Three (3) complete and collated copies of the Express Review Stormwater Management Plan

c. Three (3) collated copies of all supporting information pertaining to this project including
i. Design calculation sheets for the proposed stormwater Best Management Practices
(BMPs) sized for both on-site and off-site drainage, if any;
ii. Most current BMP Supplement completed for each proposed BMP including Design
Summary and Required Items Check List (available at
http://h2o.enr.state.nc.us/su/bmp forms.htm) along with all required items;
iii. A signed and notarized Operation & Maintenance Agreements for the BMPs, also
available at the website link in c.ii.
d. Three collated copies of full size plans with detailed construction information including, but not
limited to, the following:
i. Existing conditions of entire project area;
ii. Entire site grading plan with existing and proposed contours and delineated drainage
areas to the proposed BMPs;
iii. Erosion and sediment control plan;
iv. Detailed plan view and cross section of each BMP showing pertinent structures, zones,
pool levels, bottom and top of berm elevations, side slopes, etc;
V. Construction sequence and planting schedule for the proposed BMPs;

e. One (1) data CD of full size plans in TIFF Group 4 format (black and white, not grayscale or
color). If the plans are too large to store in TIFF format, they can be stored in PDF.
Please be advised that all Stormwater BMPs must be designed, at a minimum, to remove 85% TSS, in
accordance with the most recent version of the NC DWQ Stormwater Best Management Practices (BMP)
Manual.
Please be aware that there may be other additional conditions and requirements that must be satisfied for
projects located within Water Supply watersheds such as maximum allowable built upon area for the
entire development and stream buffer requirements. For additional information please contact your local
government watershed administrator or visit the water supply watershed website at
http://h2o.enr.state.nc.us/wswp/ and also Section 2.5 of the NC DWQ BMP Manual.
Please note that a pre-application meeting is required prior to the submittal of your completed application.
The owner of the project must be present at the pre-application meeting. The clock for this project will
not start until receipt of the completed application package and required fee. If the drainage areas exceed
what is indicated on the Initial Interest Form and/or the services are different, you may be required to
remit an additional fee.
