hydraulic capacity to treat the plant's permitted capacity.

The existing 24-mgd water treatment plant will be demolished and and a new 18-mgd water treatment plant will be
constructed which will treat water from the Yadkin River and/or Salem Lake. The new water treatment plant includes an
operations building which will house the administrative and personnel areas, maintenance shop, control room, and
chemical feed and storage; an 18-mgd basin and filter complex consisting of rapid mix

Division of Water Quality dated June 23, 2008 as shown in the attached letter. Local floodplain approval was obtained
from Jeff Kopf of the City of Winston-Salem Inspections Division dated November 28, 2007 as shown in the attached
letter. The erosion and sedimentation control plan for the project (Project ID Forsy-2008-033) was approved by Clif
Whitfield of NCDENR Division of Land Resources Land Quality Section dated March 26, 2008 as shown in the attached
letter.

Stream impact No. S3 crosses the creek at an angle less than 75 degrees in order to avoid conflict with the existing 15"
sanitary sewer line on the east side of Brushy Fork Creek and connect to the proposed Wash Water Equalization Basin on the
west side of the creek.
The structures associated with stream impacs S1 and S2 are essential for the water treatment plant improvements but were
designed to minimize stream impacts as much as possible.

In accordance with Contract Drawing B11 Note 3, the existing creek is to be intercepted and bypassed around stream impacts
S2 and S3 such that no water flows through any distrubed area until construction of these facilities is complete. Rip-rap
stabilization is shown for these structures to protect the stream-bank during and after construction.

Stormwater management in terms of detention and treatment was waived for this site because the change in impervious
area from existing to proposed conditions is minor and thus there is only a minor increase in stormwater runoff. This
waiver is documented in an email from Joseph Fogarty of the City of Winston-Salem dated February 15, 2008. The site
will include a new storm drain system similar to the existing system which discharges into the unnamed tributary (which
drains into Salem Creek) at the existing discharge location through a new Plant Outlet Structure. This storm drain system
is designed to handle the 10-year storm. Drainage area outside of the storm drain system sheet flows directly to Salem
Creek or to the unnamed tributary which drains into Salem Creek.

A new sanitary sewer system will be constructed on the site to convey all sewage generated on the site to existing
sanitary sewer manholes located on-site which are part of the City of Winston-Salem's sanitary sewer collection system.
Spent filter backwash water and filter-to-waste is treated in a wash water equalization basin and wash water clarifier and
discharged to the unnamed tributary at the Plant Outlet Structure in accordance with the NPDES permit. This treatment
process and discharge was approved by DWQ in the Authorization to Construct A to C No. 079821A02.

I have reviewed and approve the plans for the above referenced project. The site is
located off Martin Luther King Jr. Blvd near the intersection with Reynolds Park Road. The
projects consists of demolishing the old water treatment plant and re-building a new one
on the same site. The engineer has submitted a report along with the plans that indicates

A letter of request for Authorization to Construct was received February 15, 2008, by the
D LJ n, and final plans and specifications for the subject project have been reviewed and found
to tisfactory. Authorization is hereby granted for the construction of modifications to the
ex g Water Treatment Plant, with discharge of treated wastewater into Salem Creek in the
Y -Pee Dee Rive r Basin.
This authorization results in no increase in design or permitted capacity and is awarded
the construction of the following specific modifications:
Installation of a 545,285 gallon flow equalization basin, four (4) 10 Hp, 730 gpm
equalization basin pumps, a 75 ft diameter clarifier, three (3) 10 tip, 250 gpm
pumps to be installed in the existing residuals pumping station, a ten (10) ft
diameter metering manhole, expansion of the two (2) existing residuals storage
lagoons to a total of 2,08 million gallons and a new reinforced concrete and clay
liner, in conformity with the project plans, specifications, and other supporting
data subsequently filed and approved by the Department of Environment and
Natural Resources.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
Permit No. N00079821 issued July 14, 2004, a9d shall be subject to revocation unless
tewater treatment facilities are constructed in accordance with the conditions and
ms specified in Permit No. N0007982I.
The sludge generated from these treatment facilities must be disposed of in accordance
I G.S. 143-215.1 and in a manner approved by the North Carolina Division of Water Quality.

In the event that the facilities fail to perform satisfactorily, including the creation of

Upon completion of construction and prior to operation of this permitted :facility, a

WQ/DENR,1633 Mail Service Center, Raleigh, NC 27699-1633.
Upon classification of the facility by the Certification Conunission, the Permittee shall
oy a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
,water treatment facilities. The operator must hold a certificate of the type and grade at
equivalent to or greater than the classification assigned to the wastewater treatment
ties by the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each
I facility at least weekly and each Class II, III and IV facility at least daily, excluding
,nds and holidays, must properly manage the facility, must document daily operation and

A copy of the approved plans and specifications shall be maintained on file by the

This office has reviewed the subject erosion and sedimentation control plan. We find the
plan to be acceptable with modifications and hereby issue this letter of Approval With
Modifications. The Modifications Required for Approval are listed on the attached page. This
plan approval shall expire three (3) years following the date of approval, if no land-disturbing
activity has been undertaken, as is required by Title 15A NCAC 4B .0129.
Please be advised that Title 15A NCAC 4B .0118(a) requires that a copy of the approved
erosion control plan be on file at the job site. Also, you should consider this letter to give the
Notice required by G.S. 113A-61.1(a) of our right of periodic inspection to insure compliance
with the approved plan.
North Carolina's Sedimentation Pollution Control Program is performance-oriented,
requiring protection of existing natural resources and adjoining properties. If, following the
commencement of this project, it is determined that the erosion and sedimentation control plan is
inadequate to meet the requirements of the Sedimentation Pollution Control Act of 1973 (North
Carolina General Statute 113A-51 through 66), this office may require revisions to the plan and
implementation of the revisions to insure compliance with the Act.
