Dear Mr. Smith:
The Division of Water Quality (DWQ) has performed a preliminary review of a 401 Water Quality Certification
application submitted for the above referenced project. Additional information will be required in order to
complete the review process.
Please provide the following information so that we may continue to review your project.
Additional Information Requested:
1. Please explain the technique to be used for installing pipe at creek crossings. The application
does not mention whether trenching or boring techniques will be utilized.
2. Please explain the methods that will be used to conduct all work in a dry work area, when in or
adjacent to stream waters.
3. The drawings submitted with the application indicate the sewer line construction corridor within
10 feet of a stream in several locations. Water Quality Certification 3699 does not allow such
proximity with a stream. Please explain how compliance with the Certification will be met.
4. The creek crossing depicted on Drawing Numbers: C-02 does not appear to meet the required
minimum angle of 75 degrees to the streambank. Please confirm the final design angle of this
crossing.

5. Two creek crossings found on Drawing Numbers: C-02 and C-06 appear to show the final
disposition of the sewer line to be installed above the stream bottom. The original stream bottom
elevation is required to be maintained.
Please provide your response in writing to the addresses below prior to June 10, 2009. Four copies of the
required information should be provided to the 401 Permitting Unit at:

This letter only addresses the preliminary application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application are not
