enable our complete review, in accordance with N.C.G.S. 143-215.1 and 15A NCAC 2H.0105, please provide
additional or revised information to address the following:
1. Please provide in your response the capacity/number seats housed in the church.
2. Clearly list the number of bathrooms and fixtures at this facility.
3. Please explain if there is a kitchen(s)at the church.
4. Please indicate whether there is a daycare at this facility. If the church has daycare facilities, please list
the number of children that can attend and the number of daycare employees.
If no response is received within 60 calendar days [per 15A NCAC 2H.0107(b)],the project will be deactivated
and withdrawn from our review process, and DWR will recycle the documents. If you have questions, please
