Dear Mr. Jennell:
In accordance with your application for a discharge permit received on March 5, 2018, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Please note that the NCGO80000 General Permit will expire on October 31, 2018. Typically all
permit COCl are renewed automatically once the General Permit itself is finalized, unless there are
outstanding annual fees. There is no need to reapply for a renewal unless the Division directs you
to do so. Be sure to check our program website for updates and public notice of proposed renewal
actions in 2018.
Please be aware that South Buffalo Creek (segment index number 16-11-14-2) is an impaired water.
This stream is impaired because both fish community and benthos have been rated as fair. There are
also multiple Total Maximum Daily Loads (TMDLs) that affect this watershed. This information was
omitted from the original Notice of Intent (NOI) application (question 8) and should be noted in the
facility's Stormwater Pollution Prevention Plan. Information about these TMDLs can be found on
Division of Water Resources' website: https:Hdeq.nc.gov/about/divisions/water-
resources/planning/modeliniz-assessment/tmdls. In Part I, Section A, the General Permit explains that
for discharges to watersheds affected by a TMDL, coverage under this permit may depend on the
facility demonstrating no reasonable potential to violate applicable water quality standards for
specified pollutants as a result of discharges.

Please take notice that this certificate of coverage is not transferable except upon approval of the
Division of Energy, Mineral, and Land Resources. The Division of Energy, Mineral, and Land
Resources may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Resources, the Coastal Area Management Act, or any other federal or local
government permit that may be required.

NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge stormwater from a facility located at
DJ's Repair LLC
317 Edwardia Drive
Greensboro
Guilford County
to receiving waters designated as an unnamed tributary to South Buffalo Creek, class WS-
V;NSW waters in the Cape Fear River Basin, in accordance with the effluent limitations,
monitoring requirements, and other conditions set forth in Parts I, II, III, and IV of General
Permit No. NCG080000 as attached.
This certificate of coverage shall become effective May 4, 2018.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
