I’ve been in contact with Robert Tipton and have his original application for the NCGNE in West Jefferson and an
updated contact sheet. The original was signed in May of 2015. The business is Leviton Manufacturing Company, Inc. If
it’s acceptable, I’m going to do a site visit next week to see if they qualify and just go from there. BIMS shows it as in
review. They apparently never had a NCG permit.

Good Afternoon,

Back in 2015/2016 we did not keep a copy of the no exposure application – sent to the regional office once I assigned a
# in BIMS. We are just now keeping a copy of the no exposure application in LaserFiche. As of the beginning of this
year. Here’s what I found out:

NCGNE1117 was entered back in August 2016 Asheville Regional Office (address: 113 Industrial Blvd. Morganton, NC)
NCGNE1072 was entered back in June 2015 Winston Salem Regional Office (address: 618 S. Jefferson Avenue, West
Jefferson, NC)

If your contact still has a copy of these no exposure applications then I would request a copy.

I’ve copied Sue White and Glen White in the Winston Salem Regional Office since NCGNE1072 is for their region.

Laura,

Isaiah Reed in the ARO called me today. He’s following up on a No Exposure request submitted in 2016, NCGNE1117,
that is still in BIMS as “in review” status. The owner of this facility also submitted an NCGNE for a site in West Jefferson,
Ashe County (Winston-Salem’s region), but Isaiah didn’t look up the number for that one. The person who contacted
Isaiah said that he never got an inspection for these requests, and the NCGNE requests still appear to be pending.

Can you help Isaiah track down the history and next steps needed for the one in Burke County (NCGNE1117), and also
check on the one in WSRO’s region? Sounds like these may be requests that went out to the RO but never got
concluded. If I can help at all, let me know.
