Permit Renewal Request (SAW -2016-01404 / DWR#17-0023)
On behalf of the owner, Ms. Deborah R. Lewis, we hereby provide notice that the previously
permitted wetland fill impacts authorized under the attached USACE General Permit Verification
letter SAW -2016-01404 dated 2/21/17 and N. C. DWR Approval DWR#17-0023 dated January 19,
2017 have not yet occurred.
Ms. Lewis understands that the NWP and GC associated with these approvals have expired and
thereby her authorization to fill the wetlands has expired.
As such, Ms. Lewis hereby requests the renewal of these approvals under the current 2017
NWP18 and GC4139. The information and plans provided in the attached PCNapplication dated
12-30-2016 have not changed. The total requested wetland impacts remain the same at 0.098
acres.
A copy of this letter and new $240.00 DWR application fee will be hand delivered to DWR.
