In accordance with your application for a discharge permit received on February 23, 2018, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Please note that the NCG030000 General Permit will expire on October 31, 2018. Typically all
permit COCl are renewed automatically once the General Permit itself is finalized, unless there are
outstanding annual fees. There is no need to reapply for a renewal unless the Division directs you
to do so. Be sure to check our program website for updates and public notice of proposed renewal
actions in 2018.
Please be aware that Coddle Creek (segment index number 13-17-6-(0.5)) is an impaired water.
This stream is impaired because of poor fish community; however, there is no Total Maximum Daily
Load (TMDL) established for this watershed. This information was omitted from the original Notice
of Intent (NOI) application (question 8) and should be noted in the facility's Stormwater Pollution
Prevention Plan. In Part I, Section A, the General Permit explains that for discharges to watersheds
affected by a TMDL (should one be established in the future), coverage under this permit may depend
on the facility demonstrating no reasonable potential to violate applicable water quality standards for
specified pollutants as a result of discharges.

Please take notice that this certificate of coverage is not transferable except upon approval of the
Division of Energy, Mineral, and Land Resources. The Division of Energy, Mineral, and Land
Resources may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Resources, the Coastal Area Management Act, or any other federal or local
government permit that may be required.

standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,

to receiving waters designated as Coddle Creek, class WS-II;HQW waters in the Yadkin
River Basin, in accordance with the effluent limitations, monitoring requirements, and
other conditions set forth in Parts I, II, III, and IV of General Permit No. NCG030000 as
attached.
This certificate of coverage shall become effective April 30, 2018.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
