On December 20, 2017, the Division of Water Resources (Division) received CHM Homes, Inc.'s application requesting a
401 Water Quality Certification from the Division for the subject project. I have been corresponding predominately with
Marcus Barksdale regarding the project, but understand that he no longer works with Advantage Civil Engineering.
The application is currently incomplete and cannot be processed. I am waiting on the following information in order to
process the 401 request:
Approved Jurisdictional Determination from the Army Corps of Engineers.
Confirmation of the following proposed revisions to the stream impacts listed on the stream impact table in the
August 3, 2017, PCN to better reflect the Stream Culvert Replacement Exhibit, dated December 5, 2017
a. S1: Permanent 43 feet (New 68' culvert — 25' of pre-existing culvert removed = 43')
b. S2: Temporary 25 feet (Restoration of 25' of streambanks in location of removed pre-existing culvert)
c. S3: Temporary 20 feet (20 feet associated with impervious dikes for bypass pumping).
Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the
proper consideration of the application. If all of the requested information is not received, the Division will be unable to
approve the application and it will be returned. The return of this project will necessitate reapplication to the Division
for approval, including a complete application package and the appropriate fee.
